**Road to Recovery **is a program that offers comprehensive, recovery-focused acute care for individuals seeking to change their substance use. With 25 acute withdrawal management care beds (detox), we specialize in helping patients who face a high risk of clinical complications and complex health challenges.

**The role**:
Reporting to the Team Lead of the Transitional Care Unit (TCU), the **Recovery Care Worker** provides essential care and support to patients/clients accessing transitional care at Providence Health Care. This role involves observing, documenting, and reporting changes in patient/client behavior or physical status, leading group discussions on transitional care policies and substance use recovery, and facilitating leisure and social activities.

**Key Responsibilities**:

- Provide direct care by observing, documenting, and reporting behavioral or physical changes.
- Assist with patient intake, admission/discharge documentation, and entering data into computerized systems.
- Support patients/clients with daily living activities and encourage personal care tasks.
- Conduct group discussions on substance use recovery, lifestyle changes, and TCU policies.
- Promote trauma-informed, anti-Indigenous racism-free, person-centered care.
- Perform clerical tasks, including patient admissions and discharges, answering calls, and maintaining logs.
- Assist with security checks, respond to crises, and collaborate with the care team on patient care plans and referrals.
- Organize and facilitate leisure activities, such as walks, games, and art activities.
- Ensure safety by conducting searches, managing patient environments, and responding to emergencies.

What You Bring:
**Education, Training and Experience**
Completion of a social services assistant program (e.g., social service worker, mental health worker or community support worker) or an equivalent combination of education, training, and experience.
**Skills and Abilities**
- Knowledge of addiction.
- Knowledge of the principles of cultural safety and humility.
- Effective communication skills, both verbally and in writing.
- Physical ability to carry out the duties of the position.
- Ability to work independently and in cooperation with others.
- Competence in operating related equipment.
- Strong organizational and prioritization skills.
- Capability to establish and maintain rapport with patients/clients.
- Ability to observe and recognize changes in patients/clients.
- Skill in handling conflict effectively.
