Job Summary:
We are seeking a Grief Program Coordinator to join our team (12 mth maternity contract). Under the direction of the Executive Director, the Grief Program Coordinator is responsible for the delivery of services and supports to adult grief clients in Thunder Bay. This position oversees all aspects of client case management. Responsibility for case management will include the intake of client referrals (grief/bereavement), assessment and assignment/monitoring of service by volunteers, and coordination of individual and grief group supports. Case management will include the adherence to the HNW Communication Plan in order to foster strong relationships with clients

**Duties**:

- Enters referrals, required forms & assessments into database INFO ANYWHERE for all adult Grief clients in Thunder Bay
- Communicates with grief clients to determine type of service needed (Care plan)
- Matches and introduces volunteers to their grief clients and/or registers client to group services
- Maintain a grief client distribution list
- Creation and distribution of a quarterly newsletter for all grief clients
- All oversight, case management, coordination and related duties for the following programs in Thunder Bay: One to One Grief Volunteer Support Program, Hearts & Hope 5-Week Grief Support Group, Masterclass 6-Week Grief Support Group, Circle of Friends Monthly Workshops, Bi-Weekly 55 Plus Grief Support Group, Other grief events, groups & workshops including the Candlelight Memorial Service, Personal Lens on Grief, Grief Conference, Hike for Hospice, etc.
- Working directly with other roles to provide client support including the Grief Counsellor, Palliative Program Coordinator, Regional Coordinator, Administrative Support, and Volunteer Coordinator.
- Adheres to the communication plan for optimal results with assistance from Administration Assistant: Initial letter to client, Check-ins with client concerning their 1:1 volunteer, Satisfaction survey when the match is terminated, Offer all Grief support services, Thinking of You Card 1 year upon completion of services with HNW, Closing clients when they are complete service
- Directs and monitors delivery of client services to standards set by Hospice Palliative Care Ontario (HPCO)
- In collaboration with Executive Director, participate in external committees and palliative care/grief initiatives in community, to ensure representation of HNW
- Assists the Volunteer Coordinator with facilitating training among grief volunteers and provides support and guidance to individual grief volunteers (3x/annually)
- Assist the Palliative Coordinator with facilitating Grief & Bereavement and Self-Care Sessions with Caregiver Clients (2x/annually)
- Facilitates training to new Grief Facilitators and skills training to seasoned Facilitators, as needed
- Maintain clients lists of active and inactive clients, waitlists, audit lists, and related documentation as required for Accreditation by HPCO
- In collaboration with other staff, ensure that service delivery statistics are reported and data entered into our data management system in a timely fashion
- Promotion of Hike for Hospice to all Grief Clients
- Assist with the HPCO accreditation preparation and quarterly auditing
- External presentations for program promotion
- Other duties as required

Performance Review: A performance review will be held prior to the probation period of 3-months. The Executive Director will include recommendations for training and development. The process will include an opportunity for the Grief Program Coordinator to request changes to the job description, remuneration or for training.

Communications/Code of Conduct: That client focused palliative care provision requires healthy working relationships among all staff and volunteers, is a fundamental operating principle of Hospice Northwest. All staff will be responsible for maintaining positive and mutually supportive relationships within a professional environment. All staff and volunteers are responsible for exercising strict standards of confidentiality related to all client, volunteer and staff information.

Qualifications:

- Diploma and/or Bachelor's Degree in SSW or Social Work, Psychology, Gerontology, Recreation Therapy and/or a related field.
- Active registration with the College of Social Work and Social Service Work (OCSWSSW), Ontario College of Registered Psychotherapist or related College and to be in good standing is considered a strong asset, however not a requirement.
- Experience in case management, including client service, group facilitation, and planning
- Strong administrative skills with attention to detail
