South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Our mission is to improve the lives of people that face barriers to physical, mental, spiritual and social well-being. We do so by meaningfully engaging our clients and communities, ensuring equitable access to primary health care and delivering quality care through a range of evidence informed programs, services and approaches. As a leader in community health, our locally and internationally recognized community services include cutting-edge primary health services, health promotion, harm reduction, environmental health, community food centre and population-based community programs for marginalized peoples. We value health equity and inclusion and respect in our work and in the delivery of our services.
Harmony Community Food Centre (CFC), a program of South Riverdale Community Health Centre, in partnership with Community Food Centres Canada, is a welcoming place where people to come together to grow, cook, share and advocate for good food.
The role of Community Advocacy Program (CAP) Facilitator within the Harmony Community Food Centre program is to support people who live on a low income to navigate the system of social services and government entitlements and to engage and mobilize community members to take action on local issues and the larger structural forces that are ultimately responsible for food insecurity and poverty. The CAP Facilitator will support shared community meals, cooking and gardening programming and take leadership for advocacy programs that improve access to healthy food, build food knowledge and skill and support action on issues of poverty and food insecurity.

This work is rooted in strengths-based practice and an anti-oppression model. As such, the CAP Facilitator makes a special effort to engage low-income residents as program participants and volunteers including vulnerable adults, seniors, youth, and parents of young children.
