**KEY FUNCTION**:
Act as a MAID Navigator to support, educate, and advocate for individuals inquiring about or requesting the option of Medical Assistance in Dying (MAID). Facilitate the implementation, operation, and evaluation of MAID services.

**REPORTING**:
Under the general direction of the Clinical Bioethicist/Manager Spiritual and Religious Care.

**DUTIES**:

- Respond to MAID inquiries from patients and healthcare providers in a timely, non-judgmental, compassionate, and respectful manner.
- Triage MAID referrals; identify physicians to complete eligibility assessments and provide MAID.
- Ensure the MAID care pathway is in accordance with HSN policies, procedures and applicable legal and professional standards.
- Support referring physicians/Nurse Practitioners (NP), patients, family members, and healthcare teams; provide education about all aspects of MAID process.
- Encourage/support patients and families to be actively involved in decisions about their care, and to be informed of all end of life options.
- Maintain a database to produce quarterly reports on the number of MAID requests/ provisions.
- Develop and measure performance indicators to monitor quality.
- Identify gaps in support for patients and family members.
- Advocate for relevant processes/resources to improve quality of service and ensure timely access to care.
- Stay informed of new developments and emerging issues to ensure continued quality of service.
- Participate in networks to create partnerships and share knowledge/resources (e.g. Canadian Association of MAID Assessors and Providers (CAMAP) and the Ontario MAID Community of Practice).
- Navigate socio-political environments with political astuteness; negotiate through conflict and mobilizing support.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a Bachelor’s Degree or equivalent from an accredited university.
- Current Certificate of Registration in good standing with a regulatory college (e.g. College of Nurses of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers, Registered College of Psychotherapists) is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of three (3) to five (5) years’ experience working with patients who are engaging in advanced care planning and/or end of life decision making is required.
- Experience in public speaking, providing education geared to adult learners through various modalities is preferred.

- Demonstrated ability to facilitate workshops, educational programs, presentations.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.

**PERSONAL SUITABILITY**:

- Successful Vulnerable Sector Check (VSC) is required.
- Demonstrated ability to work with individuals from diverse economic, cultural, racial, and religious backgrounds.
- Demonstrated ability to work outside regular work hours as required.
