The Coordinator, Quality and Patient Safety provides support and coordinates activities within the Patient Experience, Quality and Patient Safety portfolio related specifically to the Patient Safety incident reporting system as well as the overall hospital Accreditation process. These responsibilities include timely management of safety incident reporting, managing access of the SIR system for staff, and coordinating administrative and planning functions related to Hospital Accreditation. The position serves as a liaison with other departments, including the President’s Office, Vice Presidents’ Offices, hospital staff and outside agencies. Support must be provided in a professional, timely, and effective manner.

**QUALIFICATIONS**:

- Undergraduate degree in Business, and/or health-related field required
- Minimum 2 years practical and relevant experience in quality, patient safety and/or coordination of projects
- Minimum 2 years work experience in a hospital setting
- Must have ability to problem-solve independently, anticipate needs, be proactive in preparing necessary documentation in a timely manner are required functions of the position.
- Must have proven data analysis skills and proven ability to create dynamic and effective presentations with mínimal direction or supervision.
- Experience in project management is an asset
- Experience in hospital Accreditation is an asset
- Effective communication, facilitation, and interpersonal skills
- Proven ability to work collaboratively in teams
- Must have ability and experience with hospital IT systems and electronic medical records.
- Must have ability to prepare high level presentations, diagrams, charts, visuals and to assemble information from various sources.
- Demonstrates sound use and familiarity with quality and patient safety terminology, language, and vocabulary as well as medical terminology.
- Advanced skills in Microsoft Office, including the ability to manipulate data in spreadsheets and create charts and graphs.

**RESPONSIBILITIES**:

- In consultation with the Manager of the Patient Experience, Quality, Patient Safety department:
oSupports ongoing maintenance and reporting of SIR system, develops SIR education to staff as needed regarding use of the SIR system
- oTracks Critical Incidents and prepares monthly Risk Reports for Management and Senior Leadership
- oPrepares quarterly presentation on incident analysis; this includes information retrieval, data analysis and reporting.
- oTrack and prepare Never Event reports for review and presentation as needed- Schedules urgent teleconference calls for serious critical incidents, as well as System Analysis Reviews
- Coordinates and tracks timelines for completion of incidents
- Prepares educational slides to provide information to new staff and new Managers. Training is delivered by the Manager.
- Responsible for reviewing all reported incidents in collaboration with Patient Safety/Risk Consultants
- Coordinates internal/external reviews by identifying the key stakeholders required, keeping records/tracking information and providing necessary follow-up.
- Coordination of follow-up on critical incident recommendations
- Implementation of SharePoint website to allow for transparent reporting and tracking of incidents
- Works directly with digital solutions to ensure staff have proper access to SIR system and adjusts access when appropriate
- Coordinates activities related to the organization’s preparation of Accreditation on-site survey; includes coordination of preparatory activities including tracers and follow-up items
- Resource for the Accreditation working group; prepares agenda, takes minutes, tracks action items
- Coordinates education to department leaders as it relates to quality and accreditation
- Synthesizes the self-assessment survey results and works with teams/programs to develop action-plans
- Monitors progress towards goals identified in self-assessment surveys
- Synthesize results from patient safety culture survey
- Tracks accreditation suggestions and recommendations completed by program or service
- Updates and submits preparation documents to Accreditation Canada through the Accreditation Canada portal
- Prepares for on-site survey visit including document preparation, surveyor orientation, scheduling, catering
