**ABOUT THE ROLE**
The **Provincial Virtual Walk-in Navigator** provides in the moment service planning and navigation into mental health and wellness services for children and youth experiencing complex and multiple mental health concerns, and their families to enable timely and seamless connection to needed services. The Navigator collaborates extensively with multiple stakeholders, including Brief Services Counsellors at Lead Agencies and Core Service Providers, across the province of Ontario in supporting youth and families to access the right services, at the right time closest to their home location.

Salary range:_** _$54,926.26 to $63,962.43 per annum_
- $30.18 per hour to $35.14 per hour_

- These positions will be fully remote._
This position is represented by OPSEU local 567.
- 3 positions available. 1 full-time permanent, 2 part-time permanent_
- 2 positions are designated bilingual (French and English)_
Current hours are a rotation _between the hours of 9AM - 8PM Monday - Saturday_ with the intention of expanding to increased hours and days.

**WHAT YOU WILL DO**
- Engage with child/youth and family and co-create a service plan, facilitate access to programs and services
- Ensure clients who require access to ongoing services have support in accessing the most appropriate services as close to their home as possible.
- Work with clients to understand and determine the most suitable service, agency and location to support the client and family in receiving additional services not provided by the Virtual Walk-In
- Educate families about mental health and relevant services and ensure that they understand all options available to them.
- Partner with clients and empower them to make informed choices, support service entry, transition and service discharge/re-entry planning depending on the phase of treatment and the individualized needs of the client/family
- Facilitate the next step of the client journey by contacting the selected agency to refer child/youth and family to services, review the service intake requirements, complete forms and consents required, complete any Screeners and/or Assessments required.
- Develop pathways to provincial intake models to support the transition from the virtual clinic into regional and local intake services.
- Partner with Indigenous service providers and others to ensure appropriate access and intake services are available for those clients who identify as Indigenous/Aboriginal/First Nations, Inuit, Metis.
- Connect with other service navigators and centralized intake models to share resources and best practices. Share learnings or emerging practices with team for continual improvement in system navigation and best practice standards.
- Act as an educator/spokesperson/advocate on behalf of clients and families to funders, stakeholders, intersecting sectors, partners, and others, as needed
**THE “MUST HAVES”**
The following education, professional memberships and knowledge are a requirement for this role:
- Bachelor’s degree in Child and Youth Work/Counselling, Social Work, or a related field
- Minimum 5 years of professional experience
- Experience working with electronic referral systems or electronic medical record systems
- Experience and passion for working with children and youth and their families
- Extensive knowledge of mental health disorders and diagnoses
- Familiarity with and exposure to the Diagnostic and Statistical manual (DSM)
- Ability, ease and comfort in engaging and coordinating across sectoral teams when necessary.
- Highly self-directed with excellent organizational and time management skills.
- Ability to embrace working with the families of youth, not simply the youth.
- Cultural sensitivity and an openness to consulting with and bringing in relevant cultural supports as /when needed.
- Knowledge of specialized resources i.e., Transitional Age Resources, etc.
- Excellent youth/family engagement and rapport building skills with an understanding of client-centered care.
- Advanced skills in verbal and written communication with well-developed listening skills and an understanding of motivational interviewing.
- Ability to complete required client documentation in a timely manner that adheres to PHIPA standar
