**Position Summary**:
The Indigenous Peer Support Service Coordinator within addictions and mental health expands the availability of this type of service, and it is seen as an essential component of a comprehensive and evidence-based addictions service system. This role is part of an effort to improve access to and quality of addictions services through the provincial mental health and addictions (MHA) strategy; Roadmap to Wellness. HDGH recognizes the importance of creating services that advance equity, inclusion, diversity, and anti-racism.
The Indigenous Peer Support Service Coordinator will provide addiction and concurrent disorder support and advocacy for Indigenous patients, caregivers, and their families. The Indigenous Peer Support Service Coordinator will draw on one’s own personal lived experience with addictions or concurrent disorder to assist and mentor others as they consider their unique goals and navigate how to achieve them, and offer hope of recovery for those affected by addictions.
The Indigenous Peer Support Service Coordinator will support and build relationships with individuals who are at various stages of change as it relates to addictions, with a fundamental goal of facilitating addictions treatment and achieving recovery. The Indigenous Peer Support Service Coordinator will support addiction service navigation (for example, Crisis and Withdrawal Management Services (WMS), Rapid Access Addiction Medicine (RAAM), other treatment services or aftercare supports or other peer supports. There will be a focus on supporting transition points, which may include culturally and spiritually relevant care, where requested. The Indigenous Peer Support Service Coordinator will work with agencies in bridging the service gaps between Indigenous communities and health care providers within addictions services. The Indigenous Peer Support Service Coordinator will facilitate linkages to mental health services, and other services such as chronic disease management, primary care and acute care in collaboration with local Indigenous Patient Navigators, when appropriate.
The Indigenous Peer Support Service Coordinator performs duties and responsibilities consistent with the principles of psychosocial rehabilitation, a recovery-oriented practice, and an evidence-based peer support model. The Indigenous Peer Support Service Coordinator Worker will provide information on resources and supports for recovery support. The Indigenous Peer Support Service Coordinator will raise awareness of addictions and mental health issues in one-to-one or group-based interactions. The Indigenous Peer Support Service Coordinator supports linkages that help to improve quality of life and ability to cope with daily stressors and provides crisis support for individuals as needed
RESPONSIBILITIES:

- Primary service functions include Wellness and Prevention, Assessment and Goal Planning, Communication, and Documentation
- The Indigenous Peer Support Service Coordinator utilizes one’s own lived experience and path to wellness and self-discovery to support individuals in their resilience-building process
- Model effective coping techniques and positive self-help; facilitate a strengths-based approach to care that emphasizes safety, respect, hope, self-determination and empowerment
- Assist in the development of culturally appropriate care standards, policies, procedures, practices, and hospital spaces to better support the addiction related needs of Indigenous patients, caregivers, and their families
- Ensure patient information is gathered and data is collected appropriately following organizational policy and procedure as it relates to addiction specific documentation
- Engage and recruit local Indigenous community Traditional Healers, Elders, and Knowledge Keepers to provide patient support
- Assist in providing cultural consultations and resource services and bring innovative ideas and practices to bridge traditional and western health care methods
- Monitor and record services provided for First Nation Inuit Metis and Non Status patients
- Build relationships between addiction service programs (i.e. Crisis, WMS, RAAM, indigenous specific services) and Indigenous communities based on mutual trust, respect, open communication, creative thinking, and cohesive team effort
- Provide support and consultation to addiction healthcare providers to facilitate access to addictions services or resources for Indigenous patients, their families and caregivers
