The Whatever It Takes program (WIT) is looking for WIT Facilitator WIT (Whatever It Takes) is a program for service providers who are struggling to meet their clients’ needs, due to gaps in the system and/or who need additional supports to develop individualized service plans for clients who’s highly complex needs cross multiple sectors. The WIT program services youth ages 0-18 years old.
This role is within the Complex Needs Pillar. You will report directly to the WIT and RPAC Manager and Director of WIT, RPAC and SNT.
**KEY RESPONSIBILITIES**:
- Provide clinical consultation to service providers supporting clients
- Build the capacity of community resources to support clients with complex needs
- Facilitate the development of collaborative service plans and oversee third party staff as needed
- Facilitate and chair community meetings
- Identify and address gaps in services
- Where needed, plan for WIT funding to provide individualized services and supports to respond to the complex needs of children/youth
- When developing funding proposal to support children and youth with complex needs the WIT Facilitator is responsible for the oversight of the approved budget for enhanced support and financial accountability
- Provide interim, short term coordination of services for service gap situations when required
- Maintain documentation and files as require by Lumenus in accordance with Ministry, licensing, and regulatory bodies
- Ongoing commitment to learning, understanding and incorporating knowledge of Anti-Oppressive Practice
- Ongoing commitment to expending knowledge of LGBTQ, racialized, and newcomer communities, as well as working with children/youth/adults with a dual diagnosis (mental health and developmental disability) and/or Neurodevelopmental Disabilities such as Autism Spectrum Disorder (ASD) and Fetal Alcohol Spectrum Disorder (FASD), and lived experience of racism, ableism, homophobia, or transphobia are advantageous
**QUALIFICATIONS**:
- MSW or equivalent education and experience working with complex needs individuals
- 5 years’ experience supporting individuals and families in social services environment preferred
- Experience in counseling, consultation, and advocacy
- Sound knowledge of service sectors, such as Education, Health Services (hospitals) Group Supportive Living, Child Welfare and Youth Justice
- A solid understanding of Anti-Oppressive Framework and the Bio-Psycho-Social Model, as well as clinical evidenced-informed practices such as CBT, DBT and attachment and trauma-informed work
**KNOWLEDGE, SKILLS AND ABILITIES**:
- Well-developed clinical expertise with demonstrated ability to make sound clinical judgements
- A demonstrated commitment to excellence in clinical and community centered work
- Well-developed knowledge of Toronto mental health, developmental and community services
- Highly developed interpersonal and negotiation skills in addressing contentious issues. Conflict Resolution competencies/ experience
- Extensive knowledge of physical, intellectual, and neurodevelopmental disabilities and their potential impact on individuals and family systems
- Strong decision-making skills, good judgement and the ability to navigate relationships with community partners. A capacity to build and nurture working relationships with partner agencies
- Ability to think strategically and operate from a systemic approach
- A self-starter with an ability to work independently, as well as within an established team structure

**ADDITIONAL REQUIREMENTS**:
- Satisfactory clearance under the Vulnerable Sector Screening process.
- Travel required to external agency locations (schools, hospitals, family homes, and residential settings) and family homes.
- A current Ontario Driver’s license and reliable transportation.
