**Job Summary**: The John Howard Society is set to open a new supportive housing January in the heart of Centretown, designated for adult woman who have experienced chronic homelessness.

This position is your opportunity to help establish a new housing program from the ground up It is an exciting role that requires strong leadership skills, creativity, and a passion for developing safe and resilient communities for vulnerable women. With a caseload up to 15 women, your role will be to establish strong relationships within the community, develop the implementation of client-centered care plans, to liaise with a variety of community partners to enhance support mechanisms, while also helping to foster a residential space of inclusion, safety, and belonging for all residents.

**Responsibilities**:
Clinical/Casework Responsibilities
- Complete relevant assessments to evaluate the resident’s areas of need, and to develop and implement an intervention plan to address those needs.
- Maintain knowledge of and assist clients with referrals to appropriate community service agencies
- Provide structured individual counselling with caseload at a minimum of once per week in their apartments as per Housing First best practices
- Understand individuals who struggle with addictions, aggression, physical and mental health limitations, and gender specific challenges through a trauma-informed lens
- Promote Indigenous beliefs, values, and traditions that enhance health
- Work with a client-focused, assertive case management approach to foster housing stability
- Understand and respond to the unique challenges faced by Indigenous clients
- Establish achievable, specific goals with the client, and identify intermediate targets for change
- Competency using various data collection tools and case management software
- Prepare and attend all clinical supervision sessions as required
- Maintain professional development through participation in relevant training, participation in supervision and through counselor development plans

Program/Building Responsibilities
- Intervene and provide support to client crises and basic needs
- Adhere to all security and safety protocols associated with Lisgar
- Attend staff meetings as required
- Facilitate excellent collaborative relationships with colleagues and provide coverage when required
- Assist in maintaining a positive environment for clients and colleagues
- Actively engage with the Cultural Outreach Worker and other Minwaashin staff to support clients with their housing stability and promote a healthy living environment
- Actively communicate with Support Workers to support clients and building safety
- Participate in the on-call rotation, adhering to the relevant policy and procedures
- Perform other duties as may be occasionally requested

Report Writing/ Administrative
- Maintain client master files by affixing any relevant internal or external documentation
- Ensure client file notes are maintained on the client information management system
- Produce additional written reports and records as required
- Produce all documentation associated with program research and evaluation within deadlines
- University Degree/College Diploma in a Social Services program
- Minimum (1) years’ experience in a case management role with the homeless population
- Knowledge of the history, culture, and unique needs of homeless Indigenous people
