As our company continues to experience nation-wide growth and expansion, we are looking for a Health Coach & Group Facilitator to join our newest program.

**POSITION OBJECTIVE**:
This is an exciting opportunity to play an integral role in helping to build out our new and innovative metabolic clinic in multiple locations across Southern Ontario. Jack Nathan Health’s metabolic clinic is a holistic program for patients with obesity, metabolic syndrome, and/or type 2 diabetes.

As a Health Coach & Group Facilitator, you are passionate about helping others improve their health and will provide education and support to our patients in an empathic and compassionate manner. In this role, you will be responsible for leading in-person small group meetings, customizing patient plans, helping patients track clinical outcomes, and liaising with program physicians to ensure the highest level of patient care. You will work closely with the Head of Metabolic Programming to identify areas for improvement and growth. This is an opportunity to be one of the first health coaches at Jack Nathan Health and grow a team as the program scales.

**FUNCTIONAL/TECHNICAL RESPONSIBILITIES**:
1. Lead and facilitate small, in-person group education and support sessions for patients enrolled in the program.

2. Promote a supportive and compassionate environment.

3. Provide education on dietary and lifestyle strategies that is consistent with the proprietary approach of the metabolic program.

4. Take complete health and dietary histories from patients.

5. Formulate recommendations for customization and meet patients where they are at in their journey and understanding.

6. Educate and support patients in the personal use of tools to gather health metrics such as: blood glucose monitors, blood pressure cuffs, etc.

7. Monitor patients’ health metrics on an ongoing basis to ensure the plan is achieving the desired outcomes, and flag any clinical concerns with the Head of Metabolic Programming and/or a program physician

8. Maintain patient privacy and confidentiality at all times.

9. Keep detailed clinical notes on all patients in the EMR; flag clinical concerns to the Head of Metabolic Programming and/or a program physician.

10. Take initiative and share ideas of innovative ways to develop and grow the metabolic program.

11. Utilize knowledge and learned skills to help with the training and coaching of new team members.

13. Other requirements as needed or assigned by the Head of Metabolic Programming (i.e. undertake literature reviews to aid in program development).

**INTERPERSONAL + COMMUNICATION SKILLS**

Very comfortable speaking in front of a small group and managing small group dynamics. Communicates well both verbally and in writing, shares information and ideas clearly, while actively listening to others. Focuses on creating a collaborative working environment by building strong relationships with team members. Is flexible and open-minded, soliciting performance feedback and constructive criticism. Speaks in a kind and empathetic tone, pays attention to non-verbal cues in conversations. Asks open-ended questions to gather necessary information to effectively guide patients.

**JOB KNOWLEDGE**

Strong background in nutrition and holistic health, is knowledgeable on the standards and code of ethics for front-line healthcare professionals. Actively shares their own knowledge with team members to aid in the development of other’s skills and the program. Keeps job knowledge current, looks to learn new methods and practices to expand their scope of work. Is able to utilize all tools for the position such as hardware, software, blood sugar monitoring, blood pressure monitoring, among others (training will be provided, where necessary).
