As our company continues to experience nation-wide growth and expansion, we seek a Metabolic Health Educator to join our newest program.

**POSITION OBJECTIVE**:
This is an exciting opportunity to play an integral role in helping to grow and scale our new and innovative metabolic program across Ontario. Jack Nathan Health’s Metabolic Reset is comprehensive patient education program using continuous glucose monitors (CGM) as a behaviour modification tool. Patients are taught and encouraged to select foods and habits that stabilize blood sugar to improve insulin sensitivity and drastically improve or reverse obesity, metabolic syndrome, pre
- or type 2 diabetes.

As a Metabolic Health Educator, you are passionate about helping others improve their health and will provide education and support to our patients in an empathic and compassionate manner. In this role, you will be responsible for leading in-person and/or virtual small group meetings, customizing patient plans, helping patients track clinical outcomes, and educating patients on their blood sugar fluctuations. You will work closely with the Head of Metabolic Programming to identify areas for improvement and growth. This is an opportunity to be one of the first Metabolic Health Educators at Jack Nathan Health and grow a team as the program scales.

**FUNCTIONAL/TECHNICAL RESPONSIBILITIES**:
1. Assist with lead-generation including attending trade shows, giving public talks, meeting with referring physicians and other providers

2. Hold consultations with potential new patients to assess eligibility for the program

3. Lead and facilitate small, in-person and/or virtual group education and support sessions for patients enrolled in the program.

4. Promote a supportive and compassionate environment

5. Provide education on dietary and lifestyle strategies consistent with the program's proprietary approach (training will be provided)

6. Take complete health and dietary histories from patients

7. Formulate recommendations for customization and meet patients where they are at in their journey and understanding

8. Educate and support patients in the personal use of continuous glucose monitoring (training will be provided)

9. Monitor patients’ health metrics on an ongoing basis to ensure the plan is achieving the desired outcomes, and flag any clinical concerns with the Head of Metabolic Programming and/or a program physician

10. Maintain patient privacy and confidentiality at all times

11. Keep detailed clinical notes on all patients in the EMR; flag clinical concerns to the Head of Metabolic Programming and/or a program physician

12. Take initiative and share ideas of innovative ways to develop and grow the metabolic program

13. Utilize knowledge and learned skills to help with the training and coaching of new team members

15. Other requirements as needed or assigned by the Head of Metabolic Programming (i.e. social media engagement, lead generation, undertake literature reviews to aid in program development).

Focuses on creating a collaborative working environment by building strong relationships with team members. Is flexible and open-minded, soliciting performance feedback and constructive criticism. Speaks in a kind and empathetic tone, pays attention to non-verbal cues in conversations. Asks open-ended questions to gather necessary information to effectively guide patients.
