Temporary Case Manager - Alexander Street Community

JOB SUMMARY

The Case Manager is a registered social worker who reports directly to the Project Operations Manager and works as part of a multidisciplinary healthcare team to provide a variety of treatment and case management services that are both inclusionary and flexible for individuals with mental illness and concurrent disorders (i.e. addictions), to meet the needs of the resident and to provide integration and continuity across the care continuum.

KEY RESPONSIBILITIES

The job includes, but is not limited to, the following:

- Works within and collaborates with the internal team to develop assertive case management strategies that will support the clients/resident’s well-being, health, development and community integration. Networks with community resources to facilitate appropriate referrals.
- Ensures resident referrals are followed up and supported (i.e. accompanied) as needed. Performs key duties such as, intake, mental health and/or substance use assessments treatment planning, crisis intervention, consultation, individual counselling, education, and support groups to facilitate resident’s individual well-being and integration into the community, and to coordinate resident access to services.
- Records assessments, care plans, referrals and case notes according to professional standards.
- Identifies applicable community resources for clients, contributes to the effectiveness. and development of community resources and planning with clients for care and support in accordance with their health needs.
- Within a multidisciplinary team context, provides continuity of care across the continuum by liaising with team members, developing/evaluating care plans and coordinating, implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care and supporting transition.
- Consults, confers, and collaborates with multidisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in client-centered education and research.

QUALIFICATIONS
- Degree in Social Work (BSW) plus two (2) years’ recent experience working with marginalized adults with mental health, substance use and behavioral needs.
- Current full registration with the British Columbia College of Social Workers.
- Local area travel may require the use of personal vehicle. Valid BC Driver’s License.

KNOWLEDGE AND ABILITIES
- Broad knowledge of mental illness and treatment, including principals of recovery in mental illness
- Broad knowledge of substance use, including harm reduction principals within addictions treatment
- Broad knowledge of psychopharmacology (indications and side-effects)
- Broad knowledge of counselling skills and group facilitation skills
- Broad knowledge of other community resources
- Broad knowledge of other health care disciplines and their role in resident care
- Broad knowledge of adult education principles, methods and tools.
- Broad knowledge of legislation and government policies and procedures that relate to resident.
- Demonstrated understanding and deep commitment to utilizing a trauma informed approach and evidence-based practices.
- Demonstrated understanding of and ability to work in an intensive community/residential environment, maintaining a high level of empathy and compassion while understanding and implementing compassionately informed boundaries.
- Profound understanding and appreciation of community based therapeutic endeavours, including an understanding of marginalization/alienation and its impact on well-being and wellness.
- Demonstrated listening and information seeking skills that promotes communication and lead to a co-operative approach to problem solving within a multidisciplinary setting.
- Demonstrated ability to work both independently and collaboratively as a member of a multidisciplinary team.
- Demonstrated ability to communicate effectively, both orally and in writing, with residents, their families, colleagues, physicians, and other health care staff, both one-on-one and in groups.
