Reporting directly to the Director, Outpatient Medicine and Regional Chronic Kidney Disease (CKD) Program, the Diabetes Patient Care Manager (PCM) will provide exemplary leadership to effectively manage the day-to-day operations within the Diabetes Management Program across multiple sites including: Mississauga Hospital, Credit Valley Hospital, and the Complex Centre for Diabetes Care located at our satellite site in Oakville.
- Strategic initiatives that are aligned with THP goals - quality, access and sustainability
- Quality improvement initiatives that improve patient outcomes, patient safety/relations
- Resource utilization - operational and fiscal, standardization, quality based procedures
- Promote and encourage a collaborative staff and patient environment for patient-centered care that adapts to the needs of the diverse internal and external communities

In particular, we are seeking a dynamic leader who has demonstrated the ability to think critically and strategically and lead complex cultural change in a fast-paced environment to achieve results.

**Leadership Responsibilities**
- Provide leadership to plan, coordinate and effectively manage resources (e.g. financial, human, physical space, equipment and materials) in alignment with the THP’s strategic plan
- Oversee the quality of care delivered by the inter-professional team in collaboration with the Clinical Educator(s), Program Director, Physician Chief/Medical Directors and Physician Division Heads/Site Lead
- Manage all aspects of employee relations including: human resource planning; recruitment, performance management; coaching/mentoring and professional development
- Fosters an inclusive and collaborative work environment by actively promoting EDI principles and practices
- Provides guidance and support to staff on matters related to diversity, inclusion, and cultural competence
- Support and facilitate a delivery of care model that enhances patient focused care, professional practice and accountability to achieve and/or exceed clinical practice standards
- Actively participates in committees, both internal and external, to promote collaboration and coordination of services with all stakeholders and partners
- Recognize, assess and mitigate risks related to patient and staff/provider safety.
- Implement and monitor recommendations associated with patient/employee safety incidents.
- Works with patients and families as partners in care; collaborates to continuously and proactively improve the patient experience
- Foster a culture of wellness and Joy in work within the Diabetes Management Program

**Qualifications**:

- Regulated Health Professional Baccalaureate degree required
- Master’s degree or current registration in a Graduate Program strongly preferred (e.g. Health Administration, Leadership, Quality)
- Require a minimum of five years of progressive leadership experience in clinical operations, in an ambulatory setting (preferably in Chronic Disease Management)
- Certified Diabetes Educator or extensive knowledge in diabetes management an asset
- Emotional intelligence and strong communication skills are required
- Proven success in implementation of program strategic goals including quality indicators, volume targets, and budget management
- Previous management of Regional Programs, and/or Quality Based Procedures (QBP) an asset
- Proven ability to manage multiple sites and program components
- Previous experience in working with chronic disease management programs to meet the needs of patients preferred
- Track record in innovative and successful program development preferred
- Ability to navigate ambiguity in a complex and fast-paced environment
- Proven success in managing change and transforming workplace culture
- Proven ability to manage multiple, competing priorities
- Effective team building and creative thinking skills
- Excellent organizational, communication
