**KEY FUNCTION**:
Ensure client safety by providing on site risk assessments, crisis counselling, and community connections to services for individuals presenting with mental health and addictions concerns. Work in collaboration with local police and treatment team to provide crisis resolution, divert clients from the criminal justice system and acute care.

**REPORTING**:
Under the general direction of the Clinical Manager, Counselling and Treatment, Rural Programs Mental Health and Addictions.

**DUTIES**:

- Respond in collaboration with police services United Chiefs and Councils of Manitoulin (UCCM) and Ontario Provincial Police (OPP) to potential mental health and addiction related calls.
- Assist individuals in crises within the community through crisis counselling/intervention.
- Complete mental health assessments/assess risk to the individuals in crisis and to those around them.
- Recommend measures and develop safety plans to help protect the individuals.
- Provide clinical intervention using clinical therapeutic interventions, community resources, and safety assessment tools.
- Support individuals and consult with family members and professionals regarding follow up.
- Provide transitional case management services to assist the development of self-management strategies and activities of daily living.
- Educate community/police regarding functions of the Mobile Crisis Rapid Response Team.
- Ensure accurate and timely reports, data entry and documentation in relevant systems
- Complete crisis assessments for clients in Little Current and Mindemoya Emergency sites.
- Complete intake assessments for incoming referrals.
- Provide ongoing crisis management for clients while waiting for treatment.
- Perform other duties as required.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a Bachelor’s Degree or equivalent from an accredited university.
- Current Certificate of Registration in good standing with a regulatory college (e.g. College of Nurses of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers (OCSWSSW), Registered College of Psychotherapists) is required.
- Current certification in Nonviolent Crisis Intervention is required.
- Crisis Prevention & Intervention Training is required.
- Applied Suicide Intervention Skills Training is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**KNOWLEDGE/SKILLS/ABILITIES**:

- Demonstrated evidence of practice based upon recovery principles.
- Demonstrated knowledge of the Mental Health Act and other relevant legislation.
- Demonstrated knowledge and understanding of Aboriginal Community and Aboriginal Based Services.
- Demonstrated knowledge of Manitoulin Island community resources.
- Demonstrated knowledge of the use of psychotropic medication, psychiatric diagnosis and

formulation of a mental health assessment for a diverse age group and population.
- Demonstrated training/educational experience in crisis intervention and risk assessment.
- Demonstrated ability to work collaboratively and in partnership with a multi-disciplinary team.
- Proven knowledge of the Diagnostic Statistical Manual.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated strong organizational skills with the ability to prioritize workload.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
