Looking for a community-based role in private home health care? Care At Home Services is a leading provider of innovative and compassionate home health care and community support. You will be provided training, a company car and scheduling support. We operate according to industry best practices, utilizing information technology to improve efficiencies. As a **Nurse Case Manager**, you will play a role in coordinating and providing care that is safe, effective and client centered.

**Responsibilities**:

- Collaborate with the care team to establish and maintain a safe, therapeutic environment and routine for our clients.
- Handling case assignments, draft care plans, review case progress and determine case closure
- Communicating with clients, client family members and client's medical health team to create a care plan specific to each client
- Conducting consultations for new clients and setting up care.
- Communicating and following up with staff, ensuring staff understand care plans and are comfortable with client care
- Conducting staff meetings, reviewing staff performance, and conducting annual reviews of staff.
- Dropping off and ensuring clients and staff have appropriate supplies and PPE.
- Maintaining and completing daily documentation of all client activities and events

**Salary**: $29.00-$31.00 per hour

Expected hours: 37.5 per week

- First Aid Certification (required)
