The Mississauga Halton Central Intake Program plays a pivotal role in advancing and streamlining access to a range of specialized healthcare services across the Mississauga & Halton and greater OH Central regions. As part of this program, you will be involved in overseeing centralized intake, screening, and waitlist support across several key pathways, ensuring that patients are efficiently connected to the appropriate care providers based on their specific needs. The pathways within the program that this role would be responsible for include:

Responsibilities include intake, clinical triage to assess patient needs, and coordinating care plans and resource matching to connect patients with appropriate services and providers, ensuring seamless access to diabetes management and specialized foot care to prevent and manage complications.
c) **Self-Management**: This pathway empowers individuals to take charge of their chronic conditions through education and support. It involves promoting self-management strategies for patients, offering resources that help individuals manage their health through lifestyle changes and chronic disease management.
As part of the team, you will contribute to improving access to these essential services, reducing wait times, and ensuring a seamless connection between patients and the appropriate resources. This is an exciting opportunity to be involved in a key initiative that impacts the healthcare experience across the Mississauga and Halton regions.
