Patient Care Coordinator

The Institute for Hormonal Health (IHH) is the leading integrative medical clinic in Canada specializing in the treatment of health issues that are hormonal in origin. The Institute utilizes state-of-the-art technology and an unparalleled team of medical professionals to provide the highest level of care. To achieve optimal hormonal health, we look at the whole health of the patient and treat the cause rather than just the symptoms. IHH uses a proprietary model of collaboration between Medical Doctors and Naturopathic Doctors to provide an integrative and holistic approach to balancing hormones.
As we are growing our practice, we are looking for Patient Care Coordinator to join or team As the Patient Care Coordinator, you are the primary point of contact between the patients and doctors. You will support patients during their appointments, liaison between the patient and the doctors, and are the primary contact to patients for all inquiries between appointments.

**Responsibilities**:
- Work collaboratively with physicians to provide educational consultations using superior patient care and attention to patient needs
- Qualify patients, ensure enough time is spent with patients, all patient questions are answered and diligent follow up is completed for each consultation
- Prepare and provide treatment quotes to patients
- Schedule appointments
- Assist in orders placed
- Facilitates bookings in coordination with the office manager
- Ensures patients receive all required instructions and packages after booking treatments
- Responsible for being the liaison between patient and practitioners with recommendations, prescription changes and inquiries.
- Responsible for chart review for upcoming appointments, ensuring all results are in prior to appointment
- Responsible for documenting patient discussions (appointment, inquires and all other patient interactions
**Skills and Experience**:
To be successful in this role, you are someone who’s focused on patient care and customer service and preferably have worked within or is passionate about the integrative medical field.
- Previous customer focused role and/or clinic experience
- preferably 2 years of medical office experience, including EMR or equivalent
- High attention to details
- High level of empathy and client focused mentality
- Flexible, adaptable and have an entrepreneurial spirit, that will enable you to become a driver in the team and contribute to the success of the clinic and team engagement
- Data entry and medical transcription an asset
- Strong organizational and time management skills
- Outstanding communication skills and excellent phone demeanor
- Extremely high proficiency in technology and software programs and the ability to learn new programs quickly
- Solid written and verbal communication skills
- Ability to handle confidential information
- Engaging personality and optimistic outlook
- Desire to be an integral part of a rapidly growing team dedicated to transforming the patient experience in the integrative medical world
