**Position Overview**:
The Proactive Care Coordination Lead (PCC-Lead) reports to the Quality Improvement Manager. This position is responsible for leading and managing Proactive Care Coordination Assistants (PCCAs), who work at multiple clinics across the PCN. PCCAs use the clinic’s electronic medical record (EMR) to identify and contact patients on a physician’s panel who are due for preventative health screening and chronic disease management appointments. This work is tracked in the EMR and outcomes are reported to the PCN. The PCC-Lead will ensure the PCCAs are following established, standardized processes and completing the role efficiently, with a high attention to detail.
**Key Roles and Responsibilities**:
The PCC-Lead will work closely with the Quality Improvement Manager, to implement strategic decisions related to the PCCA program.
Specifically, the PCC-Lead will
- Lead a team of 18-20 direct reports working remotely at multiple clinics
- Coordinate rotating PCCA schedules across 50+ clinics
- Manage the PCCAs, including day-to-day direction (scheduling, coverage), annual reviews, performance management, recruitment, training and education, and time management for direct reports.
- Provide support to each team member, to ensure they are utilizing their strengths effectively, are functioning at an optimal level and achieving their goal
- Continuous process improvement and efficiency boosting for the PCCA role
- Collaborate with internal and external stakeholders: Improvement Facilitators; EMR-Consultants; Primary Care Managers; clinic managers; physicians; to support successful integration of PCCAs throughout the PCN
- Collaborate with Primary Care Managers to coordinate integration of PCCAs into clinics, including arranging for Netcare access, remote EMR access, and confidentiality agreements
- Assist in the development of policies and protocols for the PCCA program
- Ensure PCCAs are adhering to the Health Information Act, and appropriate use of the Alberta Netcare portal
- Attend, and participate in, internal and external meetings as required
**Experience, Skills & Qualifications**:
- Post-secondary education with at least 5 years’ experience in a primary care setting (relevant combination of education and experience may be considered)
- Clinic management, and health care experience, especially in a Primary Care setting are assets
- Knowledge and use of primary care EMRs is required
- Experience leading teams, increasing efficiencies and managing change
- Respect and understanding of confidentiality; ability to act responsibility and accountably
- Excellent written and verbal communication skills, problem solving/critical thinking skills, and high accountability
- Analytical and problem-solving skills; ability to work on own initiative and to prioritize workload
- Awareness of strengths and limitations, professional development needs, and willingness to seek help when required
- Should behave responsibly, be accountable
