**Job Purpose**:
**Front-line service delivery function and systems change agent. Responsible for connecting with pregnant women who are street-involved, providing services that help mom become engaged in the pregnancy and stabilize, performing simple pregnancy-specific skills, general staffing of program and establishing and maintaining appropriate community contacts. Support individual capacity to take charge of their own lives and health by developing and maintaining a support relationship with users of the service, promoting risk reduction and options for behaviour change, linking clients to appropriate supports. Provide advice and support to community agencies. Document changes in the environment that affect the target population. Work with systems to make changes that support pregnant street-involved women.**:
**KEY AREAS OF RESPONSIBILITIES**:
- Outreach at Boyle McCauley Health Centre, Boyle Street Community Services, on foot outreach, and van as well as other sites as directed.
- Be actively involved in program components and activities, such as organizing hospital tours, pregnancy drop-in, etc.
- Contribute to the care and maintenance of the program's van.
- Provide orientation, supervision, support and evaluative feedback regarding student placements, Fine option volunteers, ridealongs, and community volunteers.
- Work with staff to ensure smooth referral system and resource lists for pregnancy-specific issues (non-medical)
- In co-operation with nurse and Pregnancy Support Workers, liase with community agencies with a view of encouraging their increased support role in respect to target populations and service delivery.
- Link with agencies and organizations, build and maintain relationships within the organization and with external clientel.
- Develop promotional and educational resources, including print materials.
- Provide crisis intervention as needed.
- Work with other staff to ensure that health education and counselling is available in ways that are relevant to the users of the service.
- Assist and support service users to obtain basic needs (income, food, shelter, transportation, legal and medical services)
- Case management - assess client needs and develop a mutual service plan.
- Engage in risk reduction counselling with women and families who use the service.
- Advocate on behalf of clients as required, supports individual to be a self-advocate and to use natural advocates as appropriate.
- Perform fetal heart rates, weights, fundal heights, kick counts and other basic health monitoring functions
- Provide manager with monthly reports, hours, expense claims etc. Maintain records and statistical information.
- Monitor and order supplies.
- Any other duty as directed by the program manager.
**EDUCATION & EXPERIENCE**
- Diploma or Degree in Social Services
**KNOWLEDGE, SKILLS & ABILITIES**
- Service focused mindset, ability to build and maintain professional relationships
- Full Class 5 Drivers license required
- First Aid Certification
- Training in suicide prevention and non-violent crisis intervention
- Must be able to provide a Criminal Record Check with the Vulnerable Sector and a Child and Youth Intervention Record Check
