To promote the physical and emotional well-being of Trinity Village’s Long-Term Care facility residents by organizing and coordinating users of a Resident Assessment Instrument (RAI) to gather information from residents and their families during initial and periodic follow-up interviews. The RAI Coordinator follows the Trinity Village process and procedures for clinical assessment and ensures that schedules, CQI, and reporting.

**MAIN RESPONSIBILITIES**:

- Oversee the RAI process according to home policies, MOHLTC regulation, College Standards of Practice and inter RAI requirements (see CIHI User’s Manual)
- Ensure MDS assessments, resident assessment protocols, and care planning are completed timely and in quality standards for documentation
- Adhere to Trinity Village/LHKW's Mission, Vision, and Values, and follow all organizational policies and procedures
- Adhere to the following initiatives for this role: Eden Alternative Philosophy of Care, Green Environmental Initiatives, and CARF Accreditation standards
- Choose attitudes and behaviours that are aligned with the Code of Conduct and Ethics, and ensure the health and safety of yourself and others at all times
- Actively participate in team meetings, organizational objectives, committees, and special events
- Remain compliant to all legislative bodies as required by Trinity Village/LHKW, and report as required
- Be familiar with and adhere to the Resident’s Bill of Rights as dictated by the Long-Term Care Homes Act
- Complete all mandatory training as assigned

TASKS & DUTIES:

- Audit all MDS Assessments and ensure errors are corrected prior to locking and submitting quarterly for Data Quality
- Ensure care plans and Kardex are consistently reviewed, updated, and data is current and accurate
- Ensure a resident assessment schedule is developed, completed, current, and accessible to all RAI users
- Opens MDS Assessment and sets ARD as per schedule
- Schedule and open care plans for review 2 weeks after ARD to ensure the assessments are complete and reflected accurately in the care plan
- Oversee the development and co-ordination of the data submission process
- Schedule annual Care Conferences for each resident and ensuring proper facilitation and communication with families or SDM
- Attend each Care Conference with prepared reports in order to contribute resident’s current status and ADL details
- Schedule quarterly Care Conference with the inter-disciplinary team to ensure accurate and current data collection

Qualifications:

- Current RPN or RN in good standing, registered with CNO
- A minimum of three years of clinical, project leadership, or management experience
- Minimum of one year of RAI-MDS experience with proven accuracy in reporting
- Strong oral and written communication skills
- Knowledge and experience with adult education strategies/facilitation
- Well organized, with strong time management skills
- Aptitude for detail-oriented work
- Proficient computer skills
- This position requires some evening and weekend work
