The Role:

Are you a patient focused RPN with administration experience and excellent written and telephone communication skills?

KMH Cardiology Centres Inc. is recruiting for for a Patient Care Coordinator (PCC) for our busy specialist physician practices in Kitchener.

The successful candidate will be part of a team of 10 members with the goal of facilitating timely, high quality care for patients attending KMH Clinics. The Patient Care Coordinator team works in conjunction with the Administrative Care Coordinator team to provide outstanding patient and physician satisfaction, support and retention.

The Accountabilities:

- Provides guidance, support, education and advice to patients

- Serves as a liaison between members of the multi-disciplinary team and external service providers to ensure high standards of quality and optimal management of patient care outcomes

- Coordination of in-clinic operations for physicians

- Management of Physician orders and Tasks management

- Responds to patient enquiries regarding clinical care within 24 business hours

- Inform and educate patients about tests and procedures as requested by physician

- Process prescription renewal requests as directed by physician in collaboration with administrative staff within 24 business hours

- Process physician orders in a timely manner as per Key Performance Indicators (KPI's)

- Liaise with external facilities on patient referrals on a timely basis

- Meet established KPI's for the role

- Utilize Tableau reports for daily work flow

- Update patient records and documentation with clarity and accuracy

- Collaborate with cross functional team and other KMH staff to assist with patient care

- Foster a collegial, respectful clinic environment

Minimum qualifications:

- Registered Practical Nurse with a minimum of 1-year experience

Required skills and experience:

- Proven ability to work proficiently in a busy fast paced specialist clinic environment

- Self-directed, detail oriented with an ability to multitask, establish priorities and achieve objectives with minimal supervision

- CPR

-Similar experience in administration and a clinic setting is preferred

- Strong knowledge of medical terminology

- Excellent verbal and written communication skills

- Proficiency in technology including MS Office and EMR systems experience

- Excellent time management and organizational skills

- Possesses effective analytical and problem solving skills

- Able to deal with sensitive information with a high degree of diplomacy

- Team orientation

The Patient Care Coordinator team takes pride in our patient focused environment and works very closely with service and clinical teams at KMH.

This is a 15-Month Contract role with full-time hours to cover a maternity leave absence. The role is scheduled for 5 days (37.5 hours) per week during the hours of 8.00 a.m.-4.00 p.m. Monday-Friday. Saturday's are required on rotation with the team for 1-2 times per month.

The role is based at our Kitchener lab. This position reports to the Team Lead: Patient Care Coordinators.
