**Position Summary**:
Reporting to the Manager of Patient Flow, Bed Management and Health Care Resource Team. In this role the admission RPN is responsible for efficiently and compassionately facilitating the admission process for patients, ensuring all necessary documentation is completed, and coordinating with various departments to provide a smooth transition into the hospital environment. This role plays a critical role in ensuring a positive patient experience while maintaining compliance with hospital policies and procedures. They provide excellent customer service and communicate effectively with patients, staff, and families. Responsibilities may include assisting patients with activities of daily living and measuring vital signs and practicing to their full scope of their education.

The role involves frequent interactions with patients, multiple units across the organization, healthcare providers, and support staff. The Patient Flow Admission RPN position is from 0900 to 2100 hours daily (inclusive of evenings, weekends, and holidays).

**Position Responsibilities**:

- Collaborate with bed management team on determining the flow of admitted patients through the ED to their respective units.
- Prepares patients in ED for admission to inpatient unit.
- Coordinates and actively support patient flow at time of admission to inpatient unit.
- Collaborate with healthcare providers to ensure timely patient assessments, interventions, and reassessments.
- Able to work independently, to engage and inspire staff to carry out their roles and responsibilities focusing on caring for patients, families and promoting flow throughout the system.
- Provides immediate follow-up, documentation, and communication of incidents to appropriate manager and director.
- Demonstrates excellent decision making and planning skills enabling effective time management with demonstrated ability to coordinate and prioritize numerous changing environmental demands.
- Work with patient flow and bed management team on efficient use of all available inpatient beds.
- Collaborate with ED (Emergency Department) Resource Nurse on admissions holding in ED.

- Effectively applies organizational and time management skills to prioritize workload, to problem solve, and to adapt to frequent interruptions and change.
- Works collaboratively with nursing and ancillary staff to solve problems in a fair, consistent manner.
- Contact representatives of departments not presently in the hospital for assistance with problem solving, timely and appropriately.
- Effectively communicate with patient’s and care partners about the admission process and expectation
- Function as a crucial point of contact for healthcare providers, support staff, and patients waiting for admission.
- Facilitate effective communication between nurses, physicians, and other team members to streamline patient admission.
- Adapt communication and interpersonal skills to deal effectively with patients and their families, co-workers, physician, volunteers, and others within our health care community.
- Maintains a professional and supportive leadership approach when working under pressure and in high stress situations.
