We are looking to expand our Primary Health Care team with the addition of a new Kinesiologist.

On December 15, 2021, Regeneration Community Services, Storefront Humber Inc., and The Four Villages Community Health Centre amalgamated to create the West Toronto Community Health Services (WTCHS), formally merging, and now integrating community mental health and addictions services, community support services, home care, and inter-professional primary health care services. Under one agency, our overarching goal is to provide the full range of coordinated community health and support services to help keep our clients physically and mentally healthy and living at home, in the community.

The mandate of the Primary Care division is to provide high quality, accessible primary health care to the communities within its West Toronto service area, in the context of the broader social and structural determinants of health. Primary care, clinical services and a wide range of health promotion programs are primarily aimed at seniors, families with young children, youth, and newcomers. Employees collaborate in an inter-professional team environment. The focus is on the client and capacity building. WTCHS has two locations in the west end of Toronto. It is funded by the Ontario Health - Toronto Region and is a member of the West Toronto Ontario Health Team.

You will be joining a rehabilitation team of kinesiologists, physiotherapists, occupational therapist, and massage therapists. This kinesiology position will involve the delivery of various group exercise programs in the community. The position will also involve the development of pathways/programs to link individuals to ongoing access to exercise programs in the community. You will work independently and as part of an interdisciplinary team, from framework that is trauma-informed, with a focus on anti-black and anti-indigenous racism, discrimination, and oppression. The population will be primarily older adults with various functional and mobility limitations. Services include a focus on the key determinants of health, with an emphasis on prevention, empowerment, health promotion and community capacity building. This is a unionized position.

**KEY RESPONSIBILITIES**:

- Provides comprehensive care to populations facing multiple barriers, ensuring access to those most vulnerable.
- Provide itinerant services working from different locations to create greater access to populations facing barriers to care.
- Work collaboratively within team to develop and implement group-based exercise programs in the community (primarily older adults/seniors).
- Work collaboratively with the team to develop pathways/programs to link individuals to ongoing access to exercise programs in the community.
- In collaboration with the Manager and Director, develop and implement an evaluation for group programs and pathways.
- Participate in ongoing evaluation of group.
- Supports the Centre’s student and volunteer placement programs, and supervises students and volunteers as required.
- Contributes to programs and services cycle of planning and evaluation, and participates in organization-wide planning, committees, and staff meetings as appropriate.
- Maintains and develops professional competence through appropriate continuing education methods.
- Contributes to activities to collect, analyze, and report on data and relevant information, and participates in research when appropriate.
- Safeguards client records, assures confidentiality of client information, and seeks to minimize risk.
- Identifies areas where the development of protocols and procedures is needed to improve client services or to promote more effective staff functioning.
- Contributes to efforts to secure and maximize resources for current and new programs, services, and activities.
- Complies with all relevant organizational policies, procedures, and protocols.
- Maintains all licenses, certificates, and standards related to the ability to practice as a Kinesiologist in Ontario.
- Respects and values the diversity of communities and individuals.
- Perform duties as assigned by the supervisor.

**QUALIFICATIONS**:

- Registered Kinesiologist from College of Kinesiologist of Ontario or other recognized educational institution.
- Registered and in good standing with the Ontario Kinesiologist Association (OKA).
- Experience in program planning, development, monitoring, and evaluation, and working collaboratively with external partners.
