**Position Summary**

SPARC, the Schroeder Pain Assessment and Rehabilitation Research Centre, is a clinical-research interdisciplinary program that investigates, assesses and treats chronic pain and musculoskeletal disorders. SPARC is looking for an experienced Social Worker to join this interdisciplinary team as the Social Work lead. Collaboration within the clinical and research interdisciplinary team, which includes physicians, kinesiologists, nurses, biomedical engineers and scientists, is a key component of this role.

Program planning involves developing and delivering coordinated patient care, fostering relationships with various stakeholders, and driving positive changes in service delivery, patient outcomes, and population health. Key responsibilities include evaluating patients to determine their service needs, formulating recommendations or treatment plans, implementing evidence-based intervention plans with patients and their families, and collaborating with inter-professional staff to provide patient-focused care. This role also entails facilitating referrals to appropriate community resources as needed, communicating effectively with patients and internal/external care providers, including documenting intervention plans and completing reports (e.g., third-party payer reports), and assessing

the impact of service delivery and educational programs. Participation in program planning and development, including the development of standard operating procedures and work flows, as well as engagement in professional and hospital initiatives that support quality management and research, are critical components of this position. The role includes acting as a clinical backup for colleagues when necessary, supervising students for clinical education, and committing to patient safety responsibilities and the corresponding hospital plans, policies, and procedures to ensure a safe environment for patients. Additionally, this position offers unique opportunities for research collaboration with an interdisciplinary team of chronic pain clinician-researchers, contributing to the field's knowledge base and improving care practices.

**Duties**
- Social Work program development and delivering coordinated patient care
- Assessing patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plans
- Documenting care plans and completing reports (e.g., third party payer reports)
- Implementing an evidence-based intervention plan with patients and their families, and the inter-professional team to facilitate changes in behaviour, attitudes, feelings and/or the environment
- Reassessing and modifying the intervention plan as necessary to meet the evolving needs of the patient
- Liaising with appropriate community agencies to coordinate care
- Maintaining relationships with various stakeholders and influencing positive improvement in service delivery and patient outcomes
- Supporting and participating in quality improvement initiatives at the program, departmental and hospital levels
- Participating in inter-departmental and inter-hospital rounds to speak and educate other clinicians about our program services
- Participate in case huddles and rounds to discuss patient cases with the interdisciplinary circle of care team
