Reporting to the Manager, Quality Improvement & Patient Safety you will guide, lead, implement and monitor quality improvement and project management. Working closely with Capacity, Access and Flow Program leadership, and interprofessional teams, you will be tasked with the development and implementation of processes in the delivery of services to support transitions and the discharge needs of patients. Primarily the focus will be to improve and sustain processes related to the management of care transitions, alternate level of care needs and inpatient bed utilization.

In your role as a Quality Improvement Leader you are strategically oriented in your efforts to shift culture and status quo while producing tangible deliverables. Your background has allowed you to develop a strong knowledge of operational excellence methods augmented with your ability to develop yourself and others in all project and improvement assignments. You utilize and role model communication and change management excellence in the delivery of goals and objectives. Your reliance on and expertise with structured approaches to improvement and project management is essential. Being driven by key performance indicators, scope, cost and schedule management, along with deployment monitoring and reporting, positions you as a valued resource.

DUTIES AND RESPONSIBILITIES:

- Leads the development of strategic plans focused on improving care transitions, alternate level of care needs and inpatient bed utilization, including research, data analysis, stakeholder engagement, environmental scanning, evidence based best practices and issues analysis;
- Guides, leads, implements and monitors the implementation of quality improvement activities, tracks status, and acts as a knowledge expert and resource in the development and maintenance of discharge planning quality improvement processes;
- Coordinates the delivery of appropriate education, mentoring and coaching in support of discharge planning using quality improvement methodologies;
- Provides project tools, resources and training to aid in development of project management capacity and skills amongst interprofessional teams and leaders as it relates to transition planning of the patient with complex discharge needs;
- Consults as a quality and process improvement knowledge expert and resource; and
- Other duties as assigned.

QUALIFICATIONS:

- Bachelors degree (required, Clinical Healthcare field and/or Masters preferred);
- Regulated Health Professional with a minimum of 3-5 years clinical experience (preferred);
- Possesses extensive clinical knowledge related to complex discharges from hospitals (preferred);
- Lean Green Belt, Six Sigma Green Belt or equivalent Quality Improvement certificate (required);
- Proven leadership skills;
- Demonstrated ability to organize and prioritize work;
- Demonstrated ability to develop and maintain effective working relationships with a variety of stakeholders;
- Demonstrated ability to effectively train, coach, and mentor teams and leaders;
- Demonstrated ability to facilitate project teams and improvement initiatives;
- Excellent interpersonal, communication (presentation, written and verbal), critical thinking, attention to detail and problem-solving skills;
- Experience working with large, multisite organization;
- Proven ability to act as a resource to various health professionals, such front-line staff, managers, physicians, and directors;
- Strong analytical skills including the ability to collect, analyze, share and interpret qualitative and quantitative data;
- High proficiency with the MS Office suite;
- Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices;

- Proven good attendance with the capability of maintaining the same standard.
