DUTIES AND RESPONSIBILITIES:
The Patient Care Manager, Allied Health, ARC MSK (PCM) is responsible for the management of the clinical service area and providing visible leadership to the interdisciplinary team to ensure optimal performance management and smooth operation of the service area and high quality patient care. As a transformational leader, the PCM will create a vision to guide positive change and innovation through inspiration, motivation, role modelling and coaching and by working in collaboration with the leadership and interprofessional teams in the Healthy Aging Program. The PCM facilitates and supports implementation of evidence based practice to achieve excellence in high quality delivery of patient care.

The Patient Care Manager also works closely with the Program Director, Patient Care Managers and Clinical Practice Leaders, to lead and guide specific key quality improvement, strategic and performance improvement initiatives identified as priorities in the Healthy Aging Program. Integrating strategy, quality improvement, practice and project planning across all services within the Healthy Aging program to achieve deliverables and excellence in patient care is a key component of this role. The PCM also contributes to the implementation of continuous improvement in the program to achieve excellence in person-centered care through the utilization of skills and attributes associated with the fields of process improvement, project management and change management in an integrated manner.

QUALIFICATIONS:

- Bachelor degree in a health related discipline;
- Master's degree or current enrollment in a Master's program (preferred);
- Current registration in good standing with a respective professional College in Ontario;
- Recent and demonstrated knowledge, experience and expertise in the delivery of high quality allied therapy services to a wide range of patient populations
- A keen interest and recent process re-engineering, planning and implementing experience;
- Proven track record of achieving results that improve the organization and inspire confidence and commitment;
- Demonstrated ability to lead effectively and create a high performance culture through change management strategies that facilitates growth and development;
- Demonstrated knowledge of hospital, ambulatory and community functions and current trends in health care;
- Demonstrated knowledge of relevant legislation, standards, and related best practice guidelines;
- Proven ability to problem-solve and critically analyze issues;
- Proven management skills including financial, human resources, quality improvement, project planning and risk, change management, evaluation and utilization management skills;
- Well-developed negotiation and conflict management/resolution skills;
- Effective communication (written and verbal) and interpersonal skills;

- Must have a satisfactory performance record;
- Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff safety, patient safety, environmental safety and participates in regular in-service education in this area;
