**Community Facilitator Floater Position
- 12-Month Full time Contract Position**

**Supported Independent Living (SIL), Day Program(OLC), Community Participation(CPS)**

**Education**: Graduation from a recognized college or university with a diploma or degree in a social services field, such as DSW, CYW, SSW, Psychology, Sociology, or other related studies.

**Job Requirements**: 1-2 years recent and relevant experience working with individuals with developmental disabilities and dual diagnosis. Current First Aid & CPR and C.P.I certification.

Police Vulnerable Sector Check
- current to within three (3) months. Bilingualism is an asset.

**Transportation/Travel**: A valid Driver’s License, with a **minimum** G2 and must have a reliable vehicle.

**Knowledge & Skills**:

- Knowledge of various disabilities and mental health disorders.
- Solid communication skills both verbal and written.
- Strong interpersonal skills and demonstrated ability to work independently or in a team.
- Demonstrated flexibility, creativity, and sound judgement.
- Computer proficiency in Microsoft Office Suite (Word, Outlook, Excel)

**Responsibilities**:

- Provide support to individuals in the community where they live with activities of daily living such as personal safety, healthy relationships, life skills, crisis management and prevention, medical appointments, navigating community resources and housing issues.
- Support participation with community-based and on-site activities in recreation, life skills and vocational skills development.
- Advocate on behalf of the individual and/or supporting them to advocate for themselves.
- Collaboration and/or co-ordination with families, significant others, community agencies, health care professionals of their choice to find and use services and supports to meet their goals.
- Support the development of a personal Individual’s Support Plan based on the principle’s self determination, social inclusion, citizenship, and choice.
- Understand and consistently follow where appropriate, any Behavioural, Medical and Crisis Protocols as documented.
- Support other relevant duties that may be developed or assigned.

**Job Type**: Full Time 12-month contract 37.5 hours per week, Salary $27.79, Mon to Fri but maybe required to work evenings and weekends dependent on client or program needs. Benefits include Dental Care, extended health care, life insurance, paid time off.
