**General**: Reporting to the Clinical Director, the Continuing Care Advocate (“CCA”) is responsible for ensuring a smooth transition for patients into the community through supporting the patient’s execution of their Continuing Care Plan (CCP). The CCA will engage in the near final stages of the Continuing Care Plan through discharge, Aftercare and beyond as the primary connection with Aurora. The CCA will work in conjunction with the counselling team, ARC Counselling, other continuing care providers, Admissions Coordinators and families to support, and report the patient's transition and progress to bring meaning to Recovery for Life.

**Responsibilities**:
Ensure the patient's Continuing Care Plan is complete and approved by the patient's counsellors and that arrangements are complete for all necessary Recovery Capital supports. This includes, but is not limited to a sponsor,After Care Group Sessions, counselling, therapy, medical appointments, support groups, sober living, etc.

Oversee the discharge process to ensure that all necessary paperwork, information, transportation, medications and other arrangements are complete and approved.

Ensure that the departing patient has completed the final patient survey, has been given the opportunity to provide testimonial and internet “reviews.”

Contact the patient by phone within 48 hours of discharge and each week for three weeks thereafter. The primary purpose of these calls is to gather information on the patient's adherence to the Recovery Plan, provide initial support and encouragement to the patient and direct the patient's counsellor, or after care counsellor, to follow up if significant issues have been identified. The CCA should also be advocating the patient engage in additional aftercare groups, individual, couples or family counselling if necessary.

The CCA should be an advocate of additional support that may be beneficial to the patient for the aftercare group, individual, couples or family counselling or therapy. The CCA would be responsible for notifying the Admissions Coordinator and designated counselors of any concerns regarding the patient's physical/emotional health and relapse.

Document all contacts with patients after treatment in FYI on a timely basis. Create and maintain supplemental data bases to support our management and execution of Continuing Care services to the patient.

Monitor patient's participation in Continuing Care weekly programming, Aftercare Group, counselling, therapy and any other Continuing Care Plan programs.

Participate in Continuing Care meetings.

Lead the development of suitable Aftercare Program supports in all locations where the patient lives to ensure the Continuing Care commitments we make are adequately served in a manner consistent with our brand.

Pay: $16.00-$23.00 per hour
