The **Social Service Worker** is accountable for collaborating with the departments to ensure the move-in process and experience meets all legislative requirements and Shepherd Village policies. Work with the Social Worker to provide supportive counseling, information, and referral services for residents and families. Responsible for ensuring that a family education program is in place and meeting the needs of residents, families, and Shepherd Village.

**Duties**:

- Coordinate residents moving into the long-term care home in conjunction with the Director of Care or designate.
- Provide a fulsome review of the Resident’s social history to the Staff in the home area where the Resident will reside and the interdisciplinary management team.
- Coordinates the Resident and Family orientation to Shepherd Village.
- Provides support and counseling in the initial stages of the move-in to Residents/families as identified by the assessment or by the need expressed by the Resident, Family, or Staff member or placed at the interdisciplinary Resident care conference.
- Assist with mediating any conflict between residents and their families.
- As a multidisciplinary team member, participates in ongoing care planning by involving the Resident and Family as much as possible.
- Together with the Social Worker, oversees the Resident Satisfaction annual process, including completing the survey and developing action plans.
- Develops, coordinates, and facilitates a family education program that includes internal and external speakers on topics relevant to living in long-term care.
- Attend and consult with the Family Council Chair where appropriate.
- Monitors all move-in incidents through appropriate documentation and follow-up.
- Performs quality assurance audits as dictated by the CQI program.
- Maintains daily/monthly/annual placement and social work service utilization statistics and documentation.
- Keeps Resident files up to date, including the documentation of all accommodation discussions had with Residents and/or their Family/substitute decision maker.
- Handles accommodation concerns and refers all other concerns to the appropriate department head as necessary.
- Keeps a record of all forms, information, and brochures provided to Long-Term Care Residents or provides copies to the appropriate department.
- Registered under the Social Work and Social Service Work Act, 1998 O. Reg. 79/10, s. 63
- Current membership with the Ontario College of Social Workers, Social Service Workers, Social Service Worker Diploma
- Previous experience working with seniors, working in long-term care, clinical and/or administrative capacities are definite assets or related/similar capacities
- Knowledge of the Home and Community Care Support Services (HCCSS) program related to long-term care
- Familiar with the Fixing Long Term Care Act and associated regulations
- Must possess strong computer skills (word processing, spreadsheet, database)
- Able to maintain a position of trust and confidentiality
- Must have good organizational skills when maintaining accurate and updated information and files.
- Excellent leadership, organizational and interpersonal skills
- Demonstrates initiative and the ability to work independently and as part of a team.
- Must have sensitivity and empathy, especially when working with seniors and their families
- Must be able to work flexible hours and weekends when required to meet Resident/Family and organizational needs.
