**Background**

Rexdale Community Health Centre (RCHC) is a community-based organization that provides comprehensive integrated primary health care and health promotion services across the age spectrum to the residents of Rexdale. Our approach to community health encompasses the broad factors that determine health such as education, employment, income, social support, environment and housing while utilizing community-based strategies that ensure community ownership.

**Purpose**

Rexdale CHC, is accepting proposals from a qualified strategic planning facilitator or a group of facilitators to conduct a comprehensive needs assessment to examine the current culture, social structure, barriers, gaps, strengths and assets of our community to serve and respond to the needs of Older Adult LGBTQ2+ access to Social Recreational programming.

The work undertaken will result in increased knowledge about access and the need for services and barriers to access and will lead to the creation of culturally competent social recreational wellness programming for the older adult LGBTQ2+ in North Etobicoke.

The facilitator(s) will report to the Manager, Services For Seniors, and work with the Manager, Seniors Program Coordinator and Seniors Programming Team to conduct a comprehensive needs assessment and develop a one year strategic action / work plan to include: an evaluation framework, measurable goals, objectives and outcomes.

**Responsibilities**
- To review all current RCHC materials applicable to the planning process including but not limited to RCHC Mission, Vision and Values.
- To conduct a comprehensive environmental scan.
- To define community; identify assets; collect community data.
- To coordinate key informants participation process, which may include 1-1 interviews, focus groups, surveys etc.
- To compile Needs Assessment Report
- To create a one (1) year action / work plan to include Evaluation Framework, Goals, Objectives and Outcomes.

**Proposal Submission**

If interested, please submit 3 copies of your proposal. The proposal must contain the following:

- Name of the company, name of the lead facilitator, contact information including website.
- CV for the lead facilitator and any members of the team indicating relevant qualifications and experiences.
- An understanding of the scope of the project, outlining the approach and, needed human and support resources, and timeline.
- Example of a similar or suggested work plan and critical path. Full details describing the general approach or development strategy, activity plans with time frames and explanations of how the RFP requirements will be achieved
- Three relevant references. References should include client name, contact person, phone number, project description, and results.

**Evaluation**

Proposals will be evaluated by the Director, Health Promotion, Manager, Services for Seniors and Seniors Program Coordinator. Final determination and approval will be based on the following criteria:

- Demonstrated understanding of the project.
- Clear description of elements of work including expected outcomes
- Identification of who will do the work and timelines
- Relevance of completed projects, experience, and references
- Thoroughness of proposal and ability to meet specifications.
- Proposals are to be concise and to the point

**Project Costing**

It is anticipated that the fees for the Needs Assessment and the Development of the One Year Action Plan will not exceed $8,500.

RCHC plans to complete the process by the end of March 2024.
