**POSITION TITLE**: Primary Care Coordinator

**REPORTS TO**: Manager, Primary Care

**Employment Type**: Regular Full-time, 35 hrs/wk

**Pay Scale**: $31.201/hr to $36.706/hr

Job Summary
The Primary Care Coordinator is responsible for assisting the Primary Care Manager with managing and operating the programs and services within the primary care portfolio. The coordinator works as an integrated member of the inter-professional team and is responsible for supervising the Medical Office Assistants on the team.
Job Specific Responsibilities
Clinical Administration
- Support the smooth functioning of clinical services, providing super-user support for clinical EMR systems.
- Oversee efficient and effective Medical Office Assistant practices.
- Provide quality improvement expertise to develop the portfolio Quality Improvement Plan, in collaboration with the portfolio Director and Manager, including the development of process and outcome-based measures and best practice-informed change initiatives. Provide quarterly reports.
- Oversee patient incident reporting for the portfolio, including data analysis. Provide quarterly reports.
- Develop the portfolio operating plan, in collaboration with the portfolio Director and manager. Ensure alignment with the organization’s Strategic Plan. Provide quarterly reports.
- Develop and maintain systems and regular reporting for chart audit requirements

Program Support
- Work during both regular and extended hours of operation at the PQ and SN site locations
- Maintain an awareness of current issues in the catchment area and in the health care system
- Provide support for the development and implementation of new and on-going projects and initiatives.
- Participate on committees, action teams and working groups as required
- Provide back up to the Primary Health Care Manager as required

Human Resource and Financial Management
- Provide supervision and support to the Medical Office Assistant staff.
- Conduct performance appraisals for direct reports in accordance with HR practices.
- Ensure routine information related to payroll, benefits and leaves are provided to finance and human resources staff for the direct reports
- Manage salaries and expenses for direct reports within assigned budget.
- Consult with the Manager on significant human resources issues.
- Approve routine expenditures within the approved program-operating budget as delegated by the Director and Manager.
- Development and Maintenance of Community Contacts
- Coordinate, administer or participate in programs in collaboration with other external organizations and networks

**Qualifications**:

- Undergraduate degree from a recognized university in health science, management or other related field or post-secondary diploma in related field
- A minimum of three years of supervisory experience preferred, in a community health-based setting.
- Experience with general management principles including financial and human resources
- Demonstrated ability to promote and model team work.
- Excellent communication and interpersonal skills.
- Fluency in English essential with French desirable; other languages an asset
- Ability to work flexible hours
- Experience using quality improvement tools such as Lean management, Model for Improvement, Value stream analysis, Ishikawa diagrams, PDSA cycles, control charts, measurement for QI, spread and sustainability techniques is an asset
- Strong knowledge, proficiency, and utilization of electronic medical record systems; advanced knowledge and experience with Practice Solutions Suite (PSS) is an asset.
- Experience developing performance analysis reports is an asset.
- Proficiency in the use of Microsoft 365 (Outlook, Word, Excel, PowerPoint, SharePoint, Teams).
- Energetic, responsive, reliable, flexible person and able to handle competing priorities
- Strong analytical and decision-making skills
