Job Summary:

We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in managing a caseload of diverse patients, assessing their care needs, and developing personalized care plans.

Key Responsibilities:

  • Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
  • Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans
  • Coordinate the delivery of home and community care services, including medical equipment and supplies
  • Facilitate communication among healthcare providers and patients to ensure seamless care
  • Provide regular reports to physicians, service providers, and patients as required

Requirements:

  • Registered Nurse (BScN or diploma) or equivalent degree in a related field
  • Related professional experience, preferably in community-based settings
  • Knowledge of medical diagnoses, symptom management, and cognitive/psychiatric disorders
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment and independently
