Reporting to the Manager, Patient Services of the MAiD Team, this position is a full-time Registered Nurse Care Coordinator on the Home and Community Care Support Services team that serves the entirety of the Waterloo-Wellington geography as a patient advocate, educator, and support person for individuals inquiring or choosing the option of a medically assisted death.

The MAiD Care Coordinator will be supported to work to their full scope of practice to support this patient population, as required. The MAiD Care Coordinator will work collaboratively with a broad team across Home and Community Care Support Services Waterloo Wellington including Care Coordinators, community partners, hospitals, physicians and nurse practitioners, pharmacies, retirement homes, long term care facilities and other key stakeholders.

**KEY RESPONSIBILITIES**:
Reporting to the Patient Services Manager, the incumbent will be responsible for:

- Ensuring that all patient inquiries are processed in a timely manner
- Identifying gaps in supports for patients and family members and advocating for relevant resources
- Providing education to patients and to colleagues about MAiD
- Supporting safe and seamless patient transitions between settings
- Reporting incidents or near misses to appropriate management by using TellUs
- Encouraging and supporting patients and families to be actively involved in their health care, e.g. asking questions to ensure clear understanding
- Fostering development of others including leading education sessions and Community of Practice meetings regarding MAiD
- Communicating effectively, listening well and encouraging open exchange of information
- Contributing to the development, execution and evaluation of the MAiD Care Coordination program and role at Home and Community Care Support Services Waterloo Wellington
- Purposefully building partnerships and networks to create results, connections, trust and shared meaning with individuals and group
- Demonstrating a commitment to customers and service, facilitating collaboration, cooperation and coalitions among diverse groups and perspectives aimed at learning to improve service
- Mobilizing knowledge by employing methods to gather intelligence, encouraging open exchange of information, and using quality evidence to influence action across the system

- Demonstrating systems critical thinking, thinking analytically and conceptually, questioning and challenging the status quo to identify issues, solving problems, and designing and implementing effective processes across systems and stakeholders

**EDUCATION, QUALIFICATIONS AND EXPERIENCE**:
**EDUCATION**:

- Registered Nurse in good standing with the College of Nurses of Ontario

**EXPERIENCE**:

- Minimum three to five (3-5) years related job experience working with patients who are engaging in advanced care planning and/or end of life decision making is required
- Experience in public speaking and providing education to groups is an asset

**KNOWLEDGE**:

- In depth knowledge of community based palliative care, complex and chronic diseases and population-based health is required

**TECHNICAL SKILLS**:

- Demonstrated understanding and compliance with PHIPA and FIPPA
- Valid driver’s license and access to a reliable vehicle
- The ability to travel throughout Waterloo Region and Wellington County is required

**LANGUAGE**:

- Proficiency in French is an asset.

**COMMUNICATION AND INTERPERSONAL CONTACTS**:

- Effective communication skills to establish and maintain a range of contacts with health professionals, service providers and organizations within the community
- Strong collaboration skills to participate on projects and committees with colleagues
- Ability to deliver information effectively and interpret data clearly in a variety of settings including one-on-one with staff and in team meetings
- Ability to deal constructively with a variety of contentious/difficult situations, and influence/persuade others to follow a recommended course of action
- Good coaching and communication skills to provide instructions and guidance to staff with respect to activities, challenges and questions
