**POSITION TITLE: Mental Health Case Manager (Arabic Speaking) - 1 Year Contract**

**REPORTS TO: Program Manager, Case Management Programs and Senior Manager, Case Management Programs.**

Madison Community Services (Madison) is a non-profit agency whose mission is to promote independence, health, recovery, and community integration of persons with mental health challenges through the provision of case management, supportive housing and a broad range of other programs and services.

**DUTIES, RESPONSIBILITIES, AND RELATED TASKS**

**1. Organizational**
- Assist in the planning, implementation and evaluation of the supports and services
- Comply with the agency policies, standards, guidelines and procedures.
- Participate effectively and respectfully in staff and other designated internal meetings.
- Work in cooperation with management and other staff members in the provision of efficient and coordinated services.
- Work from a recovery framework in achieving the organization’s mission, which is: _**to promote the health, independence, recovery and community integration of persons with mental health and/or addictions challenges through advocacy, education and the provision of a broad range of community based and housing support services**_.

**2. Program and Services**
- Assist in the screening and intake process.
- Complete assessments that identify clients’ needs, priorities and abilities through information obtained from the client, his/her formal and informal supports and the referral source utilizing agency and funder tools.
- Develop with each client a comprehensive individualized plan, which incorporates his/her, goals and values and identifies all skills, resources and service requirements.
- Provide direct service including: ADL skills coaching, assistance in obtaining and maintaining housing, support, counselling and feedback to enable clients to reach their goals.
- Provide feedback to the agency about client and community needs.
- Maintain confidentiality and comply with agency standards and all applicable legislation.
- Be present to cover Office Duty shifts, as required.
- Provide information and strategies for health and mental health promotion and crisis prevention.
- Assist clients in identifying and advocating for their civil, legal, housing and other rights.
- Facilitate access to desired and needed services and resources, including community supports, social recreational supports, crisis intervention and treatment, among others.
- Assist clients to identify, build and maintain a social/informal support system, where desired.
- Collaborate with community service providers.
- Participate in and contribute to internal agency committees.
- Perform duties in a safe manner, in line with Madison’s Health and Safety policies and all applicable statutes.
- May assist in the training and supervision of students and/or volunteers.

**3. Monitoring and Evaluation**
- Monitor, regularly review and, if appropriate, revise client goals based on client’s individual needs and direction.
- Evaluate achievement of goals and client satisfaction using a variety of methods.

**4. Administrative**
- Maintain client files and documentation as per agency policies, procedures, standards, guidelines and legal framework.
- Collection of data including: client demographics, service statistics, goal achievement and other systems that may be developed.
- Timely provision of reports as required or requested.

**5. Community Relations**
- Maintain relationships with networks, related professional organizations and other related service providers.
- Represent the mission, values and philosophy of Madison in the community including particular networks or meetings as requested.

**6. Professional Development**
- With the support and direction of the Program Manager, develop an annual learning plan to move toward identified performance and professional development goals.
- Participate in regular supervision with Program Manager.
- Attend relevant conferences, workshops and other in-service events as approved and/or directed.
- Participate in agency internal consultations and committees as requested
- Become fluent in Madison’s policies, procedures, standards and guidelines of operation.

**7. Other**
- Perform other related duties as assigned by the Program Manager or designate in the absence of the Program Manager.
- Hours of work are Monday to Friday, 9 am-5 pm, with occasional “on call” outside of these hours as required.
- Madison is a unionized employer and this is a unionized position; CUPE Local 3798-01.
- Travel throughout Madison’s catchment boundaries may be regularly required to provide community based supports to clients.

**QUALIFICATIONS, KNOWLEDGE AND SKILLS**
- University degree relevant to mental health & addictions case management is preferred.
- Must have a minimum of relevant diploma or certificate coupled with 3 years of experience in related adult mental health and case management.
