**Indigenous Case Manager**
**Full-Time Regular Position, 1.0 FTE (35 hours/week). Bargaining Unit - UFCW Local 175**
**Parkdale Queen West Community Health Centre** (Parkdale Queen West CHC) is a dynamic, community-based
health care organization serving mid-west downtown Toronto. Offering a broad range of services, including
primary health care, dental care, harm reduction, health promotion, counselling, and community development
programming, we are also committed to serving people who face barriers to health care, and we strive to reduce
these barriers. PQWCHC is a multi-sited organization.
**General Responsibilities**:We are currently seeking an Indigenous Case Manager to work with clients of our
Niiwin Wendaanimak Program across our Bathurst and Queen West sites. The position will report to the Manager
of Mental Health Services and work closely with the Indigenous Health Promoters and Indigenous Healer.
**Specific Responsibilities**:
1. Provides client-centered case management and coordination of services to overcome barriers to services
and to promote an equitable treatment for Indigenous clients, particularly those who use substances.
2. Provides information and referral regarding community resources.
3. Provides case advocacy to ensure that the client receives the community supports and health and social
services to which they are entitled.
4. Arranges and follows up with case consultation and case planning with other members of the health care
team at PQWCHC.
5. Collaborates with and supports the Traditional Healer in providing integrated care.
6. Utilizes Indigenous health/healing and traditional approaches in the work.
7. Integrates the socio-historical experience of many Indigenous clients, the impact of intergenerational
trauma may intersect with many struggles from a trauma-informed perspective.
8. Ensures appropriate support/treatment is delivered by maintaining client records, reports, and
correspondence within the safeguard of confidentiality.
9. Participates in the weekly Indigenous Niiwin Wendaanimak Program; this might include healing circles.
**Qualifications**:
- Undergraduate or Graduate diploma or degree in relevant discipline from a recognized college or
university.
- Three to five years of case management experience.
- Some experience providing brief counselling supports.
- Some experience engaging with various Systems (ODSP, CPP, Legal Aid, or criminal justice system,
housing systems, health systems, etc.)
- Able to assist with client accompaniments, letters of advocacy, faxing, referrals, etc.
- Experience working within Indigenous communities from a philosophy of non-interference and harm
reduction.
- Substantial knowledge and understanding of Intergenerational Trauma.
- Experience providing support to adults with significant trauma, mental health, and substance use
challenges.
- Knowledge of and experience in motivational interviewing.
- Demonstrated ability to work with multi-disciplinary teams.
- Proficiency in another language is considered an asset.
**Compensation**:$63,364 to $74,056 per annum, plus Group Benefits and HOOPP (Healthcare of Ontario
Pension Plan)
