To help fulfill its mission, LOFT is hiring a **Case Manager.**

**JOB SUMMARY**:
The transitional, _Intensive_ Case Management (ICM) Team provides direct services to homeless individuals living with mental health, physical health, and addictions challenges. The position provides both site-based services and community-based assessments and case management to a group of complex individuals who are experiencing homelessness in the City of Toronto.

The transitional, ICM works closely with The Access Point, various hospitals (emergency room and inpatient units) and other social service agencies across the City of Toronto, to help connect homeless individuals to appropriate health services. The ICM provides direct services to individuals experiencing homelessness living with complex needs. The ICM offers outreach and interim case management services to assist individuals with connecting to resources necessary to increase their quality of life. The role typically includes outreach, assessment, service-planning, linking, crisis management and program evaluation.

Service navigators, social workers and other frontline workers will identify homeless people living with complex needs - mental health, substance use as well as physical health issues - and will refer to the ICM. The individual is matched with an ICM, who will assist with connecting clients to services that will support their needs, provide assessment and transitional case management to help each individual reach their short-term recovery goals, get connected to health care supports and be connected to long term supports in their communities, to help the continuation of each individual’s journey to success. The ICM Team works closely with ICHA to help address clients’ needs comprehensively.

This position will work closely with the mental health and homeless services at the Front St Hub, and other programs within LOFT.

The position may involve driving and a valid Ontario driver’s license, access to a car and insurance might be required.

**WHAT YOU WILL DO**:

- Engages effectively with individuals experiencing serious mental health issues and/or addictions, cognitive difficulties, brain injury or primary health care issues
- Develops relationships with individuals served by the team and addresses basic and immediate needs
- Engages effectively with individuals in order to assess needs and desires for change
- Engages in crisis intervention/prevention with clients
- Assesses risk to clients themselves and others on an ongoing basis
- Assists individuals to access a range of services in the broader community
- Case manages from a psycho-social rehabilitation and recovery model, linking clients with other supports as necessary, including housing, transportation, shelter placement, identification (ID) services and translation services, among others
- Liaises with Ontario Works (OW) and the Ontario Disability Support Program (ODSP) as required
- Works from a harm reduction approach with clients dealing with addiction issues
- Advocates on behalf of homeless individuals in accessing a range of services
- Works cooperatively with team members, hospital personnel, other service providers in order to better meet client’s needs
- Facilitates transfer of cases to ongoing case management services
- Documents client-related activities accurately and in a timely fashion on the Client Information System’s database (Pirouette)
- Actively participates in regular supervision, case reviews, team meetings and training sessions
- Submits time sheets, expense claims and other required information in accordance with agency regulations
- Represents the agency at external workshops, inter-agency initiatives, staff training and other community-based activities
- Shares the driving responsibilities with other team members
- Other duties as assigned by the Program Manager or the Director
