1.18.14.1 POSITION: Program Coordinator
- Staffed Adult Supported Living Program
HOURS: 40 hours per week; This management position is out of membership of BCGEU
Purpose of Position:
Reporting to Independence Networking Services Inc. (INS) - Program Support, this position will provide individual care and guidance consistent with the needs of adults who have intellectual disabilities, medical issues, mental health issues, behavioral challenges and/or addiction issues. Provides stabilization through predictable structures, clear expectations, and positive behavior support.. Monitors program effectiveness and ensures that program delivery is in accordance with established standards and procedures. Acts as a member of the Management Team.
Scope of Position:
Implementation of support services for adults in Staffed Supported Living, families including extended family and/or significant others using the Circle of Courage Model. (The model is encompassed in four core values: belonging, mastery, independence, and generosity) and interdisciplinary approach. Support Services are tailored to the special needs of individuals building on strengths and abilities and ensuring the supports are aligned with our agency Mission, Philosophy, Vision, and Guiding Principles. Supports may be delivered individually or in groups. The range of supports will include mental health and addiction; behavioural supports; skill development; and coordination of community services.
Working with the care team, assist in the identification of barriers and solutions that will promote success for supported individuals. Support Services will be delivered with dignity in a respectful, and culturally appropriate manner.
**Responsibilities**:
1. Program Delivery - Administration
- Activities:
- Participate in recruitment, hiring, and orientation of new staff.
- Ensure all program schedules are completed and sent out to employees by the 15th of each month
- Work within a unionized environment as a manager for INS
- Organize staff training and provide orientation and support: ensure all staff are oriented according to the orientation checklist and that they have completed all required training.
- Ensure that program delivery is in accordance with established policies and procedures.
- To provide administration support as requested (i.e., annual evaluation of programs; OHS site checks, etc.)
- Oversee financial support for program needs
- Monitor delivery of medication: ensure all INS policies regarding delivery of medication are followed; ensure medications are blister packed and picked up at appropriate times; ensure all documentation is kept up to date
- Ensure referral documentation is complete
- Ensure all Individuals have an up-to-date Person-Centered Goal Plan, and those goals are functional and implemented in a timely fashion
- Implement behavioral support/plans for individuals: ensure staff are competent in implementation of behavioral and incentive plans; including written documentation and observation skills (if applicable)
- Monitor individual’s mental health: ensure staff are observing and recording mental health symptoms; attend all appointments with medical doctor, psychiatry, etc. when required.
2. Home and Community - Coordinates Delivery of Service at the Community Level
Activities:
Advocate for individuals within the mental health, criminal justice, education systems, attend appointments with adult probation, court appearances, Integrated care meetings CLBC and Mental Health.
- Collaborate with referring agency to complete intake, assessment, and goal setting for referrals.
- Maintains open communication with community resources, professionals, and agencies.
- Interacts with others involved with supported individuals to enhance their relationships within the community.
- Works closely with family members when requested by CLBC to organize and implement planning that encourage family participation.ie) family visits
- Assist families/relatives in providing care or assistance for the individual to make appropriate living plans, resolve personal adjustment issues, or cope with other difficulties when appropriate.
- Advocate for supported individuals within the family, medical system, mental health, school, community, and criminal justice systems: attend appointments, court appearances, link and collaborate with, Community Living BC, Community Corrections, and other agencies and service providers in the delivery of services.
- Ensure that up to date professional resources and best practice principles are being utilized in all aspects of communication, program development and implementation of programs.
Activities:
- Facilitates supported individual interaction to increase/enhance community inclusion
- Provision of guidance and support to assist staff to carry out duties
- Ensure staff have completed all required training to provide quality supports
- Oversee completion of PATH planning; written goal plans
