**About KFLA Regional Youth Hubs**

Youth Wellness Hubs Ontario has provided funding to expand access to youth services in Kingston Frontenac Lennox and Addington. They will be funding a Youth Wellness Team to help operate multi-service hubs for youth aged 12-25 in the region. Home Base is proud to support the Hub in joint leadership with the Maltby Centre, Kingston Community Health Centres, and Addictions and Mental Health KFLA. One Roof Youth Wellness Hub will represent the urban component of the KFLA Regional Youth Hub model. The Hubs provide an inclusive, positive, and welcoming space where participants can socialize with peers and access a wide range of wraparound supports. These services can be accessed in one location and are provided by over 20 community partners. These services include but are not limited to, mental health, substance use/addictions, primary care, crime prevention services, employment/training, housing, cultural programming, skills and well-being activities, and other community and social services.

**Duties and Responsibilities**
- Leading and/or assisting in the design, implementation, monitoring and evaluation of the program
- Work in partnership with management and staff to co-create programs with youth and either integrate them into existing program design or create new programs
- Service Oversight/coordinating of interagency collaboration
- Implementation and coordination of advisory committees
- Liaise, establish, and maintain strong working relationships with community partners
- Lead service integration including utilizing protocols and Memorandums of Understanding
- Leading and monitoring program quality assurance
- Ensuring safe work environment(s)
- Coordinating staff scheduling (for hub staff and collaborating organizations)
- Ensure clients are being triaged accordingly to appropriate resources based on need and eligibility.
- Engaging service providers to meet needs of rural youth across KFLA including in person and virtually.
- Manage and maintain the budget for the program, including programming supplies and review the budget bi-monthly with the supervisor.
- Work with the school board on Preventure implementation in the schools and in school delivery, including funding requests
- Identify additional training needs of the YWHO team and bring to the Supervisors attention
- Oversee the site implementation plan and report updates, as required, to Supervisor
- Provide transitional Case Management to youth under the age of 16 and rural youth who are looking to access services and require support

**Skills, Knowledge, and Education Required**:

- knowledge of transitional age youth needs
- knowledge of the Operating Standards for the Program and the Program’s expected client outcomes
- knowledge of the resources in the community for individuals, youth and families who are homeless, at risk of homelessness, living in poverty, or living with mental health or addiction issues and skilled at brokering services for client appropriate to the responsibilities of the job
- ability to integrate harm reduction approaches as appropriate in daily interactions with clients
- understanding of psycho-social rehabilitation, recovery model motivational interviewing and journey of change and its influence on client outcomes
- ability to identify and assess risk and implement strategies which minimize risk to self, the client, and others
- knowledge of and adherence to privacy legislation and policies within the organization related to confidentiality of client information
- strong problem solving, conflict resolution and mediation skills within a solution-focused framework
- excellent communication skills, both written and verbal
- the ability to negotiate and mediate difficult situations and produce positive results
- background in systems work, data analysis and supporting the work of others to achieve system goals
- supports a learning culture and stays current with best practices and empirical evidence in the field

**Qualifications**:

- University Degree or Community College Diploma in Social Services or related field
- 3-5 years social service or mental health experience especially with transitional aged youth
- Co-ordinating and or previous leadership experience
