To help fulfill its mission, LOFT is hiring a Relief,
Community Support Worker.

JOB SUMMARY:


As a Community Support Worker, you will be part of a team that coordinates a support network and linkages for adults with serious mental health, health, and addiction challenges.

You will provide one on one case management support for clients in the community on a daily basis.

A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect.

The role will require direct service and support to clients in area such as housing, daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual's needs.



  • Note:
Crisis services support in the Drop-in program may be required. Occasional evening and weekend and on call hours may be required. Frequent travel will be required._

WHAT YOU WILL DO:

  • Assists clients in the development and implementation of a care plan tailored to the client's needs and issues designed to improve quality of life. Identify and refer to resources specific to those needs and issues
  • Act as a liaison and client advocate among those involved with the client's care including family, other caregivers, medical and other multidisciplinary providers
  • Develop and facilitate groups designed to support life skill development. Evaluate outcomes and respond to participant feedback.
  • Provide supportive counselling or referral and linkages to health care, programs, or service providers
  • Provide crisis management/intervention and assist in the development of crisis plans for clients
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favorably on the organization.

WHAT YOU BRING:

Must Haves

  • Completion of degree in social services/human services or equivalent
  • Minimum 3 years of experience in providing support to the client population including those with mental health, health &/or addiction challenges
  • Knowledge and ability to work within a Harm Reduction framework, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities
  • Crisis intervention skills, with welldeveloped problemsolving abilities and interpersonal skills
  • Group counselling related to mental health, addictions, and related issues
  • Good oral and written communication skills and computer literacy, including MS word and Excel
  • Counselling, responding and assessment skills
  • Knowledge of community resources and supports
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies
