Reporting to the Manager of Supportive Services, the Social Worker is an integral member of an interprofessional

team and provides a range of psychosocial services to patients and their families, which may include assessment,

brief solution orientated counselling, discharge/transition planning and linkage to community resources. The

Applicants must have the following qualifications/experience:

- Honors Bachelor of Social Work degree
- Current registration with the Ontario College of Social Workers and Social Service Workers, and be in good

standing
- Minimum of 2 years experience in an acute care environment

In addition, applicants must have skill and abilities in the following elements of practice:

- Demonstrated knowledge of available community resources
- Excellent working knowledge of Consent and Capacity Legislation, Social Work Code of Ethics, Mental Health

Legislation and the Heath Care Consent Act
- Excellent interpersonal and communication skills, both verbal and written
- Demonstrated ability to work independently and in a self directed manner as well as collaboratively within an

interprofessional team
- Experience evaluating mental and emotional health, social, family and financial circumstances.
- Ability to prioritize competing demands
- Displayed abilities in critical thinking, problem solving and conflict resolution
- Proven abilities to work well under stress and in a constantly changing environment
- Experience working with people at the end of life and in the setting of bereavement

Salary Range: $35.420 to 45.130
