**Reporting to the Inpatient Clinical Manager with a Matrix report to the Clinical Manager - Indigenous Health, the Social Worker provides services to clients and/or their Substitute Decision Maker (SDM), family members, and other support persons in the client's life when applicable. From herein, the job description will refer to clients and/or others for this purpose. The Social Worker is responsible for providing comprehensive assessment and intervention that addresses the psychosocial needs of the clients and/or others. The Social Worker is an integral part of the interprofessional team, providing a social work perspective and acting as a liaison and/or consultant to internal and external programs within the scope of practice. Identifies opportunities for and participates in quality improvement activity within the care setting.**

The Social Worker (Indigenous) works within the Indigenous Health Team to carry out the delivery of culturally competent and safe client-centered care, and will practice from an Indigenous worldview that recognizes the values, beliefs and customs while working from a wholistic lens. It is the role of the Social Worker (Indigenous) to ensure clients are receiving care that is grounded in an Indigenous trauma informed framework.

The Ontario College of Social Workers and Social Service Workers acknowledges the priority of Truth and Reconciliation and issues of diversity, equity and inclusion. SJCG also commits through SJCG's Walking with Humility plan to improve relationships and practices with Indigenous people. The Social Worker (Indigenous) will work towards improving issues of systemic and institutional racism specific to Indigenous clients and families.

**Qualifications**:
Education/Experience
- Baccalaureate degree in Social Work (BSW or HBSW) (preferably focused on Indigenous Field of Study)
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Thorough knowledge of the Code of Ethics and Standards of Practice as set out by the Ontario College of Social Workers and Social Service Workers
- Knowledge of social work values and ethics, theory and established practices of clinical social work
- Understanding of the social determinants of health of indigenous people and the social consequences and risk factors associated with client conditions (ie. complex/chronic condidtions, addictions, and mental health)
- Training and experience related to client care for people from Indigenous, geriatric, addictions and mental health populations
- Knowledge of family systems and family dynamic skills in working with families
- Knowledge of community systems and resources for the purposes of networking between the facility/program and other services
- Indigenous educational background directly related to social work preferred
- Indigenous language fluency and/or Indigenous cultural education an asset
- Specialized clinical experience and/or training may be required or preferred for some positions

Skills/Abilities:

- Ability to manage a clinical practice in a health care setting
- Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
- Assessment and treatment of psychosocial issues
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to work independently, interdependently, and participate as a contributing member of a variety of teams within the workplace, community and ministries/agencies
- Strong time management, analytical/critical thinking, problem solving and decision making skills
- Ability to use independent judgment and maintain confidentiality of records, actions, and information
- Knowledge of Indigenous ceremonies and traditional healing practices
- Knowledge of Indigenous history, culture and worldview
- Knowledge of Indigenous historic and intergenerational trauma experience/impacts of Indigenous clients while utilizing a trauma informed practice through a cultural lens

**CONDITION OF EMPLOYMENT**
This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.

May be required to travel to various work locations and provide own reliable transportation
