**Health Journey Navigator**

We’re a rapidly growing sport medicine clinic that helps patients to #RiseAbove injury and return to movement. We’re seeking an Alberta registered health care provider that empowers patient results through behavioural change management. This is a new role that collaborates with physicians and patients to ensure that patients stay committed to their Patient Health Journeys, resulting in improved patient outcomes and reduced need for invasive procedures. Currently, this role is part of a pilot program that will test its effectiveness. While temporary and part-time, we expect this role to grow with us as Patient Health Journeys are the foundation of our bio-psycho-social-spiritual approach to care. **Minimum Education & Experience**
- Registered health care provider, as per the _Health Professions Act_, (e.g., LPN, etc. )
- Certified Athletic Therapist considered an asset
- Training in adult education is an asset
- 1-2 years’ experience in medical practice is preferred, including clerical functions
- Experience in behavioural change management is preferred
- Proficient with MS Office Suite (365), with intermediate to advanced skills in Excel, Word, PowerPoint, SharePoint, MS Teams, etc.

**Responsibilities**

The Health Journey Navigator (HJN) supports patients and physicians in achieving their #RISEABOVE goals through the implementation of Patient Health Journeys (PHJs). PHJs are designed for optimal patient outcomes by setting personally relevant SMART goals that guide their unique health journey. Our goal is to minimize invasive procedures by engaging patients in the journey and ensuring the plan meets their lifestyle. HJN works closely with patients and physicians to support behavioural changes necessary for patient progression and physician efficiency. Your primary responsibilities include:

- Supports patients throughout their #RISEABOVE treatment journey
- Take histories and review informed consent for in-office procedures
- Participate in formal, scheduled patient care conferencing meetings with supervising physician
- Facilitate patient care through the use of electronic medical records (EMR) including patient scheduling, data input, maintaining and completing chart notes, consultation reports, requisitions, and referral letters under the direction of the attending physician.
- Protects patients' rights by maintaining the confidentiality of personal and financial information
- Maintains operations by following policies and procedures; reporting needed changes
- Contributes to team effort by accomplishing related results as needed

**Knowledge, Skills & Abilities**
- Knowledge
- Demonstrates knowledge of privacy legislation as it pertains to both health care and business operations, as well as internal knowledge of privacy policies and procedures
- Demonstrates knowledge of Occupational Health and Safety (OH&S) and Infection Prevention and Control (IPAC) policies and procedures
- Medical terminology, including knowledge of musculoskeletal anatomy and injuries
- Skills
- Excellent judgement, problem-solving, and decision-making skills
- Excellent mediation and conflict resolution skills
- Excellent emotional intelligence (EI), team building, and interpersonal mediation skills
- Effective communication skills (written, oral, auditory), with demonstrated professionalism and etiquette using e-communications
- Highly organized with an attention to detail as it pertains to personnel records and data entry
- Adheres to assigned work schedules to complete daily tasks, and uses sound judgement to flex schedules as needed to meet corporate priorities
- Excellent record-keeping and reporting of key performance indicators (KPIs)
- Intermediate to advanced use of Microsoft SharePoint, Teams, Outlook, Word, and Excel and Microsoft operating systems
- Effective use and troubleshooting of office equipment such as computers, scanners, printers, etc.
- Ability and desire to:
