Youth Housing Based Case Manager

**Summary**
The Youth Housing Based Case Manager (YHBCM) is responsible for providing support and system navigation with the purpose of developing life skills and personal well-being for homeless and at risk of homeless youth through the YH protocol and housed with the YWCA. The YHBCM will support youth with diversion, rapid re-housing, housing maintenance and after care. The YHBCM will use client focused case management approach including harm reduction, attachment theory, stages of change, resiliency theory, motivational interviewing and trauma informed care. Facilitating access to specialized community based support services and developing programs that promote life skill development for youth are both vital duties for this role. Duties will include, but are not limited to, attending regular meetings, attending professional training workshops and continuing education to remain current with trends and best practices in the industry. The YHBCM may work within the central office or travel to meet clients in their homes and off site as needed. Other duties may be assigned as necessary.

**Job Duties**
- Provide opportunity for life skill development through one on one interaction and group activities.
- Conduct diversion, rapid re-housing and provide system navigation.
- Communicate with youth in a manner that shows respect for their life experiences, goals and concerns.
- Make comprehensive referrals and “warm” transfers as appropriate for the client.
- Conduct program intakes and orientation to all youth moving into YWCA housing.
- Meet with Manager of Housing and Youth Homelessness System Developer to discern housing placement.
- Attend case management meetings
- Coordinate and facilitate meetings with community resources to support the unique needs of the youth.
- Coordinate events and life skill development workshops.
- Support youth in establishing long term housing goals.
- Conduct life skills activities with youth
- Ensure timely completion of statistics and case management
- Identify barriers youth are facing, provide warm referrals and secure resource3s to achieve youth identified goals.
- Work in tandem with housing and youth team to deliver comprehensive, quality programming.
- Attend regular staff meetings and youth team meetings.
- Attend workshops, career development and continuing education opportunities to maintain current working knowledge of issues that affect youths and their families.
- Respect and protect the rights of clients, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity; and their right to live free from retaliation, discrimination, and abuse.
- Promote and encourage each individual to maintain an optimum level of health, well-being and self-esteem.
- Maintain and model appropriate personal boundaries with clients.
- Support clients with problem solving, compatibility, resiliency and conflict resolution.
- Promote the integration and acceptance of youth in the community.
- Respect and protect the privacy of information relating to all company clients, employees, and business affairs.
- Communicate housing concerns to Housing Manager regarding facility upkeep and maintenance.
- Understand and uphold the philosophy of the YWCA St. Thomas-Elgin.
- Maintain current knowledge and practice of all relevant plans, policies and procedures as established by the company.
- Other duties shall be assigned as required.
**Requirements**:
- Post Secondary degree or diploma in Child and Youth Work, Social Work, Addictions Counselling or relevant field required.
- More than 3 years’ experience working with youth preferred.
- Previous community related experience a definite asset.
- Demonstrated knowledge of harm reduction, diversion, and theories of change.
- Able to effectively communicate both verbally and in writing.
- High level of critical and logical thinking, analysis, and reasoning to identify underlying principles, reasons, or solutions.
- Proven organizational, time management and prioritizing skills.
- Must have own transportation to work in a variety of locations across Elgin.
**Work Conditions**
