Part-Time (21 hours/week), 3 months term contract with the possibility of term extension_
**JOB SUMMARY**:
- The Home and Community Care Worker is an integral member of our Home and Community Care team. In this role you support Tsleil-Waututh Nation community members by maintaining their ability to continue living at home safely. A Home and Community Care Worker assists with clients’ personal care, monitors health changes, performs various housekeeping tasks, meal preparation, grocery shopping and companionship activities for clients in their homes.**DUTIES AND RESPONSIBILITIES**:
**Assist with personal care as per care plan including but not limited to**:
- Bathing, toileting, oral care, skin care, hair care, dressing, nail care
- Feeding and mobilization
- Rehabilitation exercises
- Medication reminders
- Health advocacy
- Observe, document, and report changes in the client’s physical condition, behavior or appearance
**Assist with the activities of daily living including but not limited to**:
- Light Housekeeping - including but not limited to vacuuming, dusting, dry mopping, dishwashing, cleaning the kitchen and bathroom, and changing beds
- Laundry and linen service to include washing, ironing and folding clothes
- Prepare meals and snacks according to care plan - this may include following a specialized diet plan as identified on the care plan
- Assist with shopping for grocery and clothes
- Escort clients on appointments, including doctor visits, friends, family members, or stores
- Monitor client supplies
- Provide companionship for the client. This includes accompanying client on walks, trips to social/recreational activities, assistance with hobbies, etc.
**Assist the Home and Community Care team**:
- Participate in team and department meetings as client assignments permit
- Provide support to the Public Health and Home and Community Care RNs, i.e. appointment bookings
- Assist in organizing community health events, i.e. mass vaccination clinics, health and wellness fairs
- Collaborate with external health care and social services partners
**QUALIFICATIONS**:
- Health Care Assistant Certificate preferred.
- Grade 10 or equivalent plus a minimum of one (1) year of related experience in house cleaning, maintaining healthy environments, direct home support program delivery
- May consider alternate combinations of relevant education and work experience.
- CPR, first-aid and food safe certifications
- TB screening
- A valid BC driver’s license and reliable transportation is an asset
- Must successfully pass a Criminal Record Check
- Must be fully vaccinated against COVID19 (minimum of 2 doses)
- Previous experience working in a First Nations community and/or organization is preferred.
**KNOWLEDGE, SKILLS, & ABILITIES**:
- You are a people person, with demonstrated skill in effective oral and written communication, interpersonal relations, and conflict resolution. You are able to work independently, and in collaboration with others. In addition, you have the physical ability to carry out the duties of the position. Flexible, kind and genuine, you enjoy working in a dynamic environment and are able to adapt to changing needs and priorities by way of effective organization and time management. You have a strong ethic of client service, and are eager to learn about the culture and traditions of diverse Indigenous Nations.-
**WORKING CONDITIONS**:
- ** Physical Demands**
- Physical activity for outdoor education, physical education, as well as in-class. Need to be able to walk for sustained periods, and able to navigate uneven terrain.
- Manual dexterity required to operate a computer and A/V system.
- ** Mental Focus**
- May deal with youth who can, from time to time, be demanding and challenging. Must be able to remain patient and committed to teaching, and may have to engage in conflict resolution or crisis management at times.
- May have to manage a number of projects at one time, and may be interrupted frequently to meet the needs of youth and organizations.
- ** Environmental Conditions**
- Interacts with community members, employees, management and the general public.
- Noise levels may escalate during activities, events and outings.
- Need to be comfortable being outdoors and leading outdoor activities.
