We are looking for our next new addition in the unique role of Community Care Planner. This role works with families in the community, understanding care options, completing client assessments, building Care Plans and hiring, developing and nurturing field staff. This role also supports community engagement, local brand awareness, business development in the Greater Vancouver Region. Must have a valid driver’s license and access to a personal vehicle. Must be available for an on-call rotation of approximately 1 week per month. References required.

Responsibilities as Community Care Planner
- Conducting client assessments and developing Care Plans, including in-home assessments
- Acting as a resource to client families
- Building the Care Team
- Prioritizing client and caregiving staff satisfaction
- Participate in the initial screening and hiring of caregiving staff
- Supervising, managing and motivating the Care Team members
- Relationship development with Clients and their families.
- Engage directly with the Right at Home management team, business development representatives, and other staff as an integral part of the company’s goal to continue to be Canada’s leader in the delivery of home health care services
- Working closely in collaboration with Care Coordinator/Scheduler.
- Schedule and conduct meetings with various referral sources in the community to develop business opportunities and expand footprint within designated areas
- Conduct exceptional follow-up to your referral source meetings and meaningfully grow those relationships.
- Assist in achieving an exceptional experience within our Right at Home system through warm, positive communication, encouragement, inclusiveness and leadership.
- Be an excellent brand ambassador for the Right at Home

Essential Functions - Direct and Delegated Responsibilities
- Work from the Vancouver office during weekdays (9am to 5pm).
- Travel across the Greater Vancouver region is an expected and anticipated part of your role. Conducting meetings, consultations, and engagements will happen in the broad region.
- Drive some marketing strategy, social media strategy, business development, business growth and other communication channel strategy for brand and services promotion.
- Schedule and conduct meetings with various referral sources to provide awareness of, and promote, Right at Home supportive care services, and meaningfully grow those relationships.
- Monitor and attend relevant events for community-awareness opportunities.
