**Overview**:
In accordance with the mission, values and principles of Covenant House Toronto and the needs of young people, the Manager, Case Management Services (CMS) provides leadership and oversight of case management services to a diverse group of young people with complex care needs. The Manager, CMS leads a professional case management team of Registered Social Workers and ensures that services delivered are outcomes focused, clinically sound and in keeping with best practices, responsive to the needs of young people, consistent with evidence informed practice and compliant with agency policies and procedures. The Manager, CMS is responsible for ensuring that people & culture practices are consistent with agency policies and compliant with relevant legislation. As a member of the program management team, the Manager, CMS is responsible for managing strategic initiatives as assigned. Working collaboratively with the program management team, the Manager, CMS has a key role in building the capacity of program staff to understand and utilize Trauma Informed and Harm Reduction practices integrating an anti-racist, equity, diversity and inclusion lens. The Manager, CMS also plays a lead role in managing service partnerships and various initiatives within the sector and wider service community.

**Responsibilities**:

- Responsible for ensuring that services delivered to youth are consistent with Covenant House principles, are outcomes focused, clinically sound and consistent with evidence informed practice (Trauma Informed Care, Harm Reduction and EDI frameworks)
- Ensures that the case managers develop case plans in collaboration with young people based on the goals that young people identify. Responsible for playing a key role in collecting and monitoring relevant statistical information, data driven decision making and addressing quality assurance and quality improvement related to case management services.- Responsible for ensuring that services provided to young people are co-coordinated with other support service areas
- Responsible for identifying service and sector trends, gaps in service and areas of program development
- Plays a lead role in the creative development of new services in response to identified trends, emerging needs
- Participates in the strategic planning process identifying and implementing objectives for program areas in their portfolio Leads communities of practice to advance case management practices.- Actively contributes to the Program Management team- Supports other program managers and supervisors by offering direct consultations, trainings, and coaching- Responsible for managing service partnerships and various initiatives within the sector
- Ensure that all people & culture policies are adhered to consistently throughout all program areas of responsibility
- Provides leadership, sets expectations, collaborates and promotes effective working relationships across program departments
- Supervises, appraises, and evaluates staff in the case management department.
- Promotes a healthy team culture and supports continuous learning and professional development of the case management team members.
- Responsible for hiring, appraising, and terminating of all direct report staff, working with the Manager, People & Culture and consulting with the Director as needed.
- Identify training needs of staff, collaborate on relevant trainings to staff and recommend training sessions
- Provide leadership around team development for these program areas
- Follow organization health and safety policies and programs
- Ensures compliance with The Ontario College of Social Workers and Social Service Workers and that case managers are in good standing with the College.

**Qualification, Experience**:

- B.S.W. or M.S.W plus minimum 5 years related working experience, including 2 years at a supervisory/program management level.
- Registration in Ontario College of Social Workers and Social Service Workers is a requirement
- Demonstrated experience supervising a team of social workers/case managers and multi-disciplinary teams.
- Training in and experience using, implementing, and training staff in trauma informed care, and harm reduction practices integrating an equity, diversity, and inclusion lens.
- Demonstrated experience with quality improvement projects and/or program evaluation is an asset.
- Demonstrated leadership, initiative, critical thinking, and conflict resolution skills.
- Demonstrated skills in human resource management.
- Excellent written and oral communication skills and computer literate.
- Able to make decision on a timely basis.
- Demonstrate experience of SSAH Shelter Support Housing Administration
