**Position Description**:
The **Patient Care Information Coordinator** will be responsible for the patient scheduling process and facilitate the delivery of patient care by providing a high level of support and assistance to patient cluster staff and patient/families. They will support the operation of the team/unit by providing clerical and administrative functions in a proactive and organized manner.

**Here's What You'll Get To Do**:

- Responsible for coordination of the patient referral/consult processes. Ensure related documentation is complete and notifying appropriate individuals
- Responsible for coordinating and booking diagnostic tests and follow up appointments to best meet child/family needs
- Maintain appointment schedules within EPIC and notify patients/families, and team of scheduling changes, follow up and cancelled appointments
- Ensure related scheduling documentation is processed, organized and available
- Answer phone calls and respond to requests for information from families and health care staff
- Ensure that the patient/families and the health care team are informed of related scheduling documentation
- Work collaboratively to maintain consistent program delivery, cross training and support/back up
- Work in collaboration with other Patient Care Information Coordinators in the Haematology/Oncology Clinic to provide administrative support related to patient care activities
- Identify issues and needs related to patient flow, administrative processes and technologies and take the initiative to problem-solve independently or with others
- Acts as liaison between patient care area and other departments/units across the Hospital and with community institutions as required

**Here’s What You’ll Need**

**Essential Requirements**
- Minimum of 3 years clerical or administrative experience in a health care setting.
- Understanding of & familiarity with medical terminology
- Excellent customer service skills, strong interpersonal, communication, and organization skills
- Demonstrated ability to remain focused and prioritize tasks in a busy and high stress environment
- Ability to work a variety of scheduled day shifts Monday through Friday

**Assets**
- Post-secondary administrative/health care diploma preferred
- Experience in a hospital environment
- Previous scheduling experience
- Experience with Epic

**Employment Type**
- Full-time, Monday - Friday, 35 hours per week
