SALARY RANGE: $26.22-33.24 per hour

**SUMMARY OF THE POSITION**:
The Assertive Community Treatment Team (ACTT) is a multidisciplinary team that provides comprehensive, community-based psychiatric treatment, rehabilitation, and support to persons who experience serious and persistent mental illness or concurrent issues that are complex and cause significant functional impairments.

**The Housing Support Coordinator will**:

- Provide support coordination as the primary case manager for a caseload of clients
- Provide housing related support - obtaining and maintaining housing
- Assist in linking clients with community resources and providing consultations to other team members
- Supportive counselling and crisis support
- Medication administration
- Support with Activities of Daily Living (ADLs)
- Harm reduction support

**HOURS OF WORK**:
The Housing Support Coordinator will work thirty-five (35) hours per week, with the hours being flexible to the needs of clients and program. The individual will be required to work a minimum of one (1) evening per week and two (2) weekend shifts every ten (10) weeks.

**ESSENTIAL QUALIFICATIONS**:

- Post-secondary degree/diploma in human services/human sciences or related field
- Minimum three (3) years’ experience within the community mental health field working with adults recovering from serious mental illness, addiction or supportive housing
- Experience in supporting individuals experiencing acute crisis and where possible taking action that is consistent with the consumer’s crisis plan of action
- Ability to advocate and liaise with housing providers, landlords, boarding homes, shelter operations and operators of other housing facilities
- Have knowledge of the Residential Tenancies Act (RTA) and the Landlord Tenant Board (LTB)
- Sound knowledge of community resources and the ability to liaise with community agencies, generic services, mental health professionals, specialized health care, recreation and leisure opportunities and vocational programs
- Proficient in the use of Word, Excel, electronic record keeping and data collection systems as well as CRMS
- Strong assessment skills and demonstrated ability to assess complex situations and make appropriate decisions independently
- Certification in First Aid/CPR, Non-Violent Crisis Intervention and suicide risk assessment preferred
- Must have a valid Ontario driver’s license, relevant insurance and use of a reliable vehicle
- Able to successfully complete and provide a current Police Vulnerable Sector Check
