**Job Description**:

Reporting to the Director, Patient Flow and Care Transitions, the Manager, Community Access is responsible for all aspects of planning, organizing and directing services that support access and transitioning of clients through the healthcare system. The Manager, Community Access applies a broad knowledge of the health care system-to-system processes that will support a smooth transition of clients through various programs within Home & Community Care and Long-Term Care Services.

The Manager, Community Access researches, develops and implements system change processes enhancing access and transitioning throughout the service continuum. The Manager, Community Access is responsible for the development and implementation of business decisions and processes to maximize system efficiency.

The Manager, Community Access supports and facilitates flow of patients into the community such as home care, care centres, convalescent programs, end of life areas, community hospices and community transition units, and is responsible for developing over capacity planning in conjunction with leaders in Acute Care, Home & Community Care and Long-Term Care Services. In collaboration with other leaders, the Manager, Community Access develops strategies for capacity mitigation such as winter bed planning and monitors service pressure bed capacity across Island Health.

**QUALIFICATIONS**:
**Education, Training And Experience**:
A level of education, training and experience equivalent to a master's degree in a relevant health discipline and seven (7) years' relevant experience, including five (5) years' related work experience in a complex organization including experience and/or training in Systems Thinking.

**Skills And Abilities**:

- Formal project management training is an asset.
- Exceptional communication skills and the ability to lead and positively influence others.
- Demonstrated skills and abilities in oral and written communication, public speaking, conflict resolution, negotiation, mediation and crisis intervention.
- Ability to facilitate teamwork and collaboration with diverse groups.
- Ability to balance and adapt to changing priorities and workload pressures.
- Ability to use software and computer programs to develop quality management reports.
- Physical ability to perform the duties of the position.
- Ability to travel and use of personal vehicle for business purposes.
- Ability to integrate quality improvement methodologies into daily operations to ensure and improve patient safety and experience.
