**Altum Health / UHN Medical Solutions**

This role falls within UHN Medical Solutions, a division of Altum Health focused on non-rehabilitation revenue generating work. UHN’s tele-monitoring program, which is utilized at all UHN hospital sites and within four provinces with external hospital partners, has demonstrated improvement to key indicators associated with patient safety, support for nurses and other health professionals, as well as realized significant budgetary savings. There is considerable potential for UHN to bring its tele-monitoring innovation to other health sectors (e.g. Long-Term Care), and to hospitals throughout the province, country, and across the globe.

This role will focus on supporting the expansion of tele-monitoring at UHN and at external hospital partner sites. In particular, this role will drive clinical relationships with external hospitals, helping to, among other things, refine tele-monitoring processes for external clients, developing strategies with external sites to increase utilization of tele-monitoring, refine standards and SOPs, and act as a primary point of contact for all clinical requirements and issues related to external party service provision. The role will also lead the development, implementation, evaluation and maintenance of quality improvement initiatives both at UHN and at external partner sites. Support identifying and engaging new partner sites will also be a core component of the role.

Reporting to the Director Business Development, and working closely with the Tele-Monitoring Executive Clinical Lead, the Clinical Program Manager is a key member of our leadership team that can listen, collaborate, connect the dots, and build trust in a team. They are able to think strategically, act practically and implement judiciously all while building commitment to a collective goal.

**RESPONSIBILITIES INCLUDE**:

- Drive operations related to external clients - updating and relating processes for external clients, developing strategies with external sites to increase utilization of tele-monitoring
- Engage with external client staff in meetings to establish and manage on an ongoing basis, tele-monitoring standards and SOPs and related practices
- Along with program executive clinical lead, act as the primary point of contact for all clinical requirements and issues related to external party service provision
- Lead the development, implementation, evaluation and maintenance of quality improvement initiatives which are in alignment with established UHN standards
- Monitor quality improvement outcomes on a regular basis; develops action plans to address identified issues
- Deliver orientation and in-servicing for external clients
- Support end-to-end ownership of multiple project components throughout project life cycle within a multi-disciplinary team environment, including monitoring the UHN deliverables and obligations to each external partner site
- Investigate and follow-up on formal incident reports and safety concerns. Develop and implement plans to address safety concerns and strategies to evaluate success
- Maintain a strong focus on change management and communications across the multiple stakeholders with an emphasis on customer service
- Regularly interface with project sponsors and stakeholders at various levels to build and maintain strong relationships for successful project and program delivery
- Regularly monitor stakeholder engagement and report on implementation progress, risks and issues to Director on a regular basis and identify strategies to minimize/resolve

**QUALIFICATIONS**:

- Undergraduate Degree is required in a recognized nursing designation
- Masters degree in a recognized clinical field is a asset
- Project Management Professional (PMP) or equivalent designation and Change Management Certification are assets
- Minimum 6 -10 years clinical experience
- Demonstrated track record of successfully delivering large, complex projects, preferably in a public-sector or healthcare environment with multiple stakeholders is preferred
